Evergreen Court Adult Day Services, Woodbury, is open! So happy to see our gang and welcome new participants.

Evergreen Court is an exciting daytime program ideal for the medically challenged adult. The program provides physical, social and mental stimulation in a safe, supervised environment. All precautionary measures are taken to ensure COVID resistance. Individualized care promotes an optimal level of functioning to facilitate good health and safety in the community.

Evergreen Court strives to make a positive difference for the participant, family, and caregiver.

In addition to being a provider for Medicaid, the JACC and Statewide Respite programs, we are proud to have been awarded the Older Americans Grant and the AADSP (Alzheimer’s Adult Day Services Program) Grant to financially assist those needing services.

The Older Americans Grant is ideal for those having a short term need for services, those managing a crisis situation or those needing assistance while seeking other financial assistance Criteria for eligibility includes being age 60 or older, being a resident of Gloucester County and requiring the services of an adult medical day program.

The AADSP (Alzheimer’s Adult Day Services Program) Grant benefits both the participant and the caregiver. The Grant provides for a “break” for the caregiver while ensuring care to the participant. The co-pay for the participant is based on a sliding scale determined by their income. Criteria for the AADSP Grant includes: 1) the participant must have a diagnosis of Alzheimer’s Disease or a dementia-related illness which is permanent and progressive, and documented by a physician; 2) Be routinely supervised by an informal caregiver not receiving remuneration; 3) Be a resident of New Jersey; and, 4) Have documented evidence of income to meet eligibility.
