It usually starts with that first step in the morning.

You swing your legs out of bed, put your foot down… and feel that sharp heel pain.

Or maybe it is not the heel. Maybe it is that deep ache in the ball of your foot after a short walk, the kind that makes you cut errands short.

A lot of our patients tell us the same thing. “I thought it would just go away.” Sometimes it does.

But sometimes the real issue is that the natural cushioning under your foot has thinned over time. Less padding means more pressure, and that pressure adds up.

This is one of those injuries you don’t plan for.

She was outside clearing icicles off her roof after the snow. A large piece of ice broke loose and came straight down onto the top of her foot.

Instant swelling. Immediate bruising. The top of her foot turned black and blue pretty quickly. There was also a small cut underneath the bandage.

We took X-rays, and sure enough, there’s a small fracture across the distal phalanx. The good news is it’s not displaced, meaning the bone pieces are still lined up the way they should be. That makes a big difference.

Because it’s stable, she doesn’t need surgery. No pins. No screws. Just protection and time.

She’s wearing a surgical shoe to limit motion and protect the area, and she’ll stay in that for about four more weeks since we’re one week out from the injury. We’ll follow up to make sure it’s healing properly.

Sometimes fractures aren’t dramatic sports injuries. Sometimes they’re just bad luck and a chunk of ice from a roof.

If you drop something heavy on your foot and notice swelling, bruising, or pain that doesn’t improve, it’s worth getting an X-ray. Not every fracture looks obvious from the outside.

This is something we see more often than people realize.

This patient has neuropathy, which means he doesn’t have normal feeling in his feet. He wore a different pair of shoes for just one day. Inside the shoe, there was a small piece of plastic sticking out near the sole.

For most people, that would have hurt immediately. He would have taken the shoe off right away.

But because of neuropathy, he couldn’t feel it.

By the end of the day, that small area of rubbing had already started turning into the beginning of a foot ulcer on the side of his toe.

That’s the danger with neuropathy. It’s not just numbness. It’s the loss of the early warning system that pain normally gives you.

The good news is this was caught early. There are no signs of infection. We’re treating it conservatively with antiseptic care, protective gauze, and most importantly, offloading the area with a wider shoe so there’s no pressure while it heals. It should take about two to three weeks.

This is exactly why daily foot checks are so important if you have neuropathy. Look at the sides of your toes. Check inside your shoes. Feel for seams, rough edges, or anything that could rub.

Small injuries can become big problems when you can’t feel them happening.

Do your corns and calluses keep coming back, no matter how many times you file them down?

Corns and calluses usually aren’t random. They’re your skin’s way of saying, “This area keeps getting too much pressure.”

Sometimes it’s the shoes you wear every day without thinking about them. A sneaker that’s a little narrow in the toe box.

Work shoes that are stiff in certain spots. Heels that shift your weight forward. Even flats that look supportive but don’t actually absorb pressure.

Your skin responds by thickening to protect the deeper tissue. That’s how calluses form. Corns are smaller and more concentrated, often on or between the toes, and they can be surprisingly painful because they press inward.

The frustrating part is this: If the pressure doesn’t change, they usually come back.

That’s why simply shaving them down at home doesn’t always solve the problem. The real fix often involves better shoe fit, softer materials, wider toe boxes, or sometimes orthotics to redistribute pressure more evenly.

If you’ve been dealing with the same spots over and over, it might be worth having someone look at how your shoes and foot structure are contributing.

Bring your everyday shoes with you. You might be surprised how small changes can make a big difference.

If you’re living with diabetes, your feet deserve extra attention.

Not because you’re doing anything wrong, but because diabetes changes how your body heals.

We’ve seen it many times: a small blister from a shoe, a tiny cut, or a cracked heel that seemed harmless. What would normally heal in a few days can quietly turn into something much more serious.

That’s because diabetes can affect both circulation and nerve function.

When blood flow is reduced, healing slows down. When nerve damage, called neuropathy, sets in, you may not feel pain the way you used to. That means you might not even notice an injury right away.

And small injuries, when unnoticed, can lead to:

- Foot ulcers that don’t heal
- Infections that spread quickly
- Hospital stays or surgery in severe cases

The tricky part is that it often starts small.

If you’ve ever felt like you’re walking on a pebble that never goes away, this might sound familiar.

This patient had a very painful callus under her first metatarsal, right at the ball of the foot. The real issue wasn’t just the callus. It was fat pad atrophy, which means the natural cushioning under that area has thinned out.

So every time she stands or walks, there’s basically bone pressing against the ground without enough padding. That’s why it hurts.

After numbing the area, we injected a small amount of Liposana, which is an injectable fat product designed to add cushioning back where it’s been lost.

In her case, we placed about 1.5 cc right under that pressure point to help restore some of that natural shock absorption.

Once we finished, we placed a small sterile strip over the injection site and carefully padded the area to offload it. She’ll wear a surgical shoe and limit pressure to the heel for a few days so the injected fat can settle right where we want it.

The goal is simple:
Less pressure. More cushioning. Less pain with every step.

We’ll check on her in two weeks to see how she’s feeling.

If you’ve got hip pain or mobility issues and you can’t comfortably reach your toes, this is for you.

I had a patient with a suddenly painful toe and he had no idea why. Turns out his toenail got so long that it dug into the side of the skin. It caused a deep cut and bleeding.

Not because he ignored it, but because he physically couldn’t get down there to see what was going on.

That’s the part a lot of people miss. Long nails can:

- Cut into the skin
- Cause bleeding and sores
- Turn into a bigger problem fast

If you can’t safely trim your own nails, don’t force it. Get regular help, either a pedicure you trust or a podiatry visit.

If one leg feels longer than the other, you’re not imagining it. It can happen after knee or hip surgery, or even from back issues, and it can throw your whole body off.

A lot of people try a heel lift insert first, but here’s the problem: it eats up space in your shoe and your heel starts slipping out.

A better option (when the difference is bigger) is a shoe lift added to the outside of the shoe by a cobbler. They cut the sole and build a lift into the full length of the shoe, so it sits and feels way more stable.

It’s usually around $60, and honestly, that’s cheaper than walking “off” every day and paying for it in your feet, knees, hips, or back.
