This patient came in with that super specific kind of pain at the tip of the second toe, the kind that makes you change how you walk and suddenly every shoe feels like a bad idea.

Here’s what was really happening:
Her toe was being pulled downward by a tight tendon underneath, so the tip kept taking all the pressure. Not a “tough it out” situation. Just a mechanical problem doing what mechanical problems do.

So we did one of my favorite quick in-office fixes: a flexor tenotomy.

It’s simple:
- clean the skin
- tiny release of the tight tendon
- then we test it right away by having her try to curl the toe

And when that toe finally relaxes and straightens? You can see the relief.

No stitches. Just a small strip over the spot, and it heals up on its own.

Quick clinic-office unboxing moment. 👟

Shoutout to Steve at Sneakarama for dropping these off after his recent meeting with Ultra. I’m unboxing them right here in the office before heading home.

First thing I notice? The toe box.
Ultra does a solid job giving your toes room to spread instead of squeezing everything forward like a lot of running shoes do.

These have a bit of stack to them and feel more like a stability-style running shoe. A lot of running shoes right now are trending toward a higher heel profile, and Ultra is clearly playing in that space with this model.

That said, the toe box is still doing what it should. And for me, that matters more than almost anything else.

I’ll give a full opinion once I’ve actually worn them and put some time in, but if you have a wider forefoot, deal with bunions, or constantly feel cramped in standard sneakers, this style is worth paying attention to.

People are always surprised when we say this, but this is what a dancer’s foot actually looks like.

This patient is a semi-professional ballet dancer who spends a lot of time en pointe. And when you look closely, you can see how much her feet work for her.

Calluses on the tips of the toes from constant pressure.
Thickened toenails and darkened areas underneath from repeated impact.
A small bunion starting to form.
A blister that needed protection just so she could keep dancing.

Most of this is normal for dancers.

What wasn’t normal was the pain.

Under one of her calluses, there was a fluid-filled sac (a bursitis) that was incredibly tender. From the outside, it looked like “just another callus.” But when we pressed on it, it was clear that something deeper was going on.

We treated the painful area with a combination of radial shockwave therapy, focused shockwave therapy, and laser treatment.

She’s halfway through a six-session plan, and already noticing improvement. We’re also helping her rethink her pointe shoes, including options with built-in padding or silicone liners to reduce pressure where her foot needs it most.

The goal isn’t to stop her from dancing.
It’s to help her dance without unnecessary pain and prevent small issues from turning into bigger ones.

If you’re a dancer and something “small” on your foot keeps bothering you, that’s worth paying attention to. Pain is information, not a weakness.

Next thing you know, your ankle feels stiff, your knees feel cranky, and your lower back is doing this whole “why am I involved?” thing.

If you’re dealing with a metatarsal fracture, here’s the part most people don’t want to hear but need to hear:

Even when it “doesn’t feel that bad,” a fracture still needs time.

In general, most fractures take at least 6–8 weeks to heal. That includes metatarsal fractures (the long bones in the middle of your foot).

And yes, that timeline is true even if you’re tough, busy, and really good at pushing through pain.

But here’s what can make it take longer:

If the fracture goes into a joint surface (the cartilage area where bones move against each other), healing can slow down because that area is constantly in motion.

If you’re still walking on it without enough protection, even “just a little,” you can keep re-irritating the bone and delay healing.

If healing takes longer than expected, that’s called a delayed union. It doesn’t mean you’re doomed, it just means you need better support or a different approach.

What treatment usually looks like:
✅ A walking boot (most common now)
✅ Sometimes a surgical shoe
✅ Less commonly a cast
✅ And in some cases, staying off the foot completely for a bit

And if the bone is being stubborn: There are more advanced options like a bone stimulator (uses electronic waves to encourage healing) and even shockwave therapy in the right situation to help jumpstart the process.

One more thing that matters more than people realize: Your body’s healing tools. Things like vitamin D, bone quality, and overall health can change how fast you recover.

If you’ve been told “6–8 weeks” but you’re still hurting, still swelling, or still limping, that’s a sign to get it re-checked instead of guessing.
