Wurtland Nursing and Rehabilitation We provide a warm, welcoming environment and highly individualized skilled nursing care. Our Care Team is specially-trained to work with seniors.

Located in the small town of Wurtland, Kentucky, Wurtland Nursing & Rehabilitation offers compassionate 24-hour Skilled Nursing care in our 126-bed facility. We listen, we understand, and develop personal therapy and care programs designed to maximize your abilities and life. This includes assisting with daily living activities and skills, such as balance training, medication management, IV therapy, and diabetes management. Should you need support services, know that we can arrange everything from mental health consultations to x-rays, lab work to podiatry services, as well as private home and hospice care.

🌟 Congratulations to Trent Kozee, our Care Team Member of the Month at Wurtland Nursing & Rehabilitation! Trent joined us in May 2025 as a Maintenance Assistant and stepped into the role of Maintenance Director in August. Since then, he has been a true blessing—making sure everything runs smoothly, from lights and water to heat and comfort. Thank you, Trent, for your hard work and commitment to excellence! 💙❤️

Since joining our team as a CNA in September 2025, Josh has shown incredible dedication and compassion. He understands the wants and needs of our residents and is always ready to jump in and lend a helping hand.
