Stable community housing can be particularly difficult to secure for geriatric individuals with complex behavioral health needs.
Mount Rogers Regional Geriatric Team, in partnership with DBHDS, has been open 3.5 years. During that time, the team has accepted 276 individuals as state psychiatric hospital discharges or as diversions from state psychiatric hospitals. Only 5 of those individuals (1.8%) have been re-hospitalized.
Our team has successfully discharged individuals to many community settings, such as the Geriatric Transition home, various nursing homes, assisted living facilities, and back to their homes.
Regional Geriatric Team staff are trained in Positive Approach to Care, and work with nursing homes and assisted living facilities to support individuals and staff and keep individuals in their communities where they can age in place.
