Your doctor and nurse will work together with you to create a plan made just for you and is coordinated with other health care providers. Our goal is to provide excellent primary medical care and we want to make it as easy as possible for you to receive that care. We want to help you prevent sickness and live your life to its fullest. If you are unhappy with your care in any way, you may request to talk to your nurse or to a member of the Family Medicine's management staff. If you belong to a managed care insurance company that pays for your primary care services, they will give you a card that identifies your Primary Care Provider (P*P). The person who is identified as your primary care provider will be an Attending physician who will be providing your care. However, all members of the healthcare provider team, including nurses, registrars and other doctors work closely together. APPOINTMENTS
We offer convenient appointments to meet your busy schedule, and if necessary, we can arrange same day appointments for urgent situations. When we make appointments, we try to arrange a time when it is easy for you to come in. Please come 20 minutes before your appointment to register, and call us as soon as possible if you do not want to keep an appointment or if you want to change the time of an appointment. When you come in, please bring your Family Medicine photo ID card and medical insurance card with you. HOURS OF OPERATION

If you are sick and need to be seen, you can come in without an appointment. If there are many people waiting to be seen, the nurse will examine the people who are sickest first. That means we can’t always predict how long your waiting time will be. However, all patients who need to be seen by a healthcare provider will be. NON-EMERGENCY MEDICAL NEEDS
If you have a question regarding your health, please call Family Medicine during office hours at 914.623.5400 and ask to speak to your nurse. EMERGENCIES
A Family Medicine doctor is available 24 hours a day, 365 days a year, for our patients. To reach us after hours, call the Family Medicine at 914.623.5400. Tell the answering service you are a Family Medicine patient and want to speak to the doctor on call. Please make it clear that it is an emergency. If you are having chest pains or other symptoms of a heart attack, or if you have serious injuries, please call 911 immediately. PRESCRIPTION RENEWALS
If you only need your current prescription renewed, please call the Family Medicine nurse at 914.623.5400 and ask for a renewal. If you call in before noon, you may pick up your prescription later that day. Otherwise, it will be ready the following day. Please note that Family Medicine does not fill prescriptions; you must take your prescriptions to your pharmacy to receive your medication. INSURANCE/FEES
We accept many commercial insurance programs, as well as the government-financed programs Medicare, Medicaid, Family Health Plus and Child Health Plus (see details below). For patients without insurance and inability to pay, we provide financial aid/charity care and can set fees based on each patient’s gross family income. CHILD HEALTH PLUS,

Families with low incomes who do not qualify for Medicaid may qualify for low-cost health insurance through two New York State programs: Family Health Plus, which covers adults, ages 19-64 and Child Health Plus, which covers children under 19. The insurance covers immunizations as well as medical care.
