FDA has issued warning letters to Target, Walmart, Kroger, and Albertsons after recalled ByHeart Whole Nutrition infant formula was found on store shelves following the Nov. 11 recall expansion.

FDA and state partners conducted more than 4,000 retail checks nationwide and found recalled formula in over 175 locations across 36 states. As of Nov. 26, FDA reports no additional findings.

As of Dec. 10, 51 infants across 19 states with suspected or confirmed infant botulism have been reported with confirmed exposure to ByHeart formula. Investigations into the root cause are ongoing, and FDA says it cannot rule out contamination affecting all ByHeart products. CDC has expanded its case definition to include any infant exposed since the product’s release in March 2022.

Eligible applicants must:
