LifeStream has four unique communities across the valley, with a variety of living arrangements and services including Independent Living, Assisted Living, Memory Care, Long-term Skilled Nursing and Short-term Rehabilitation. Our unique and homelike settings, programs and services have continually pushed forward over the years, always incorporating industry best practices. They have been further designed to not only serve our Residents, but also to educate Residents’ families and the community at large on the value of Seniors in our society.
