After 18 years of business, we announce the permanent closure of Performance Physical Therapy on October 20, 2023. Over the years, it has been our privilege to serve the Siskiyou County community by providing exceptional care and support to all who walked through our doors.
We want to express our deepest gratitude to patients who have trusted us with your health and well-being. Your loyalty and support have meant the world to us, and it has been an honor to be a part of your journey toward better health and recovery. To our referring healthcare providers, we would like to thank you for your confidence in our practice to refer your patients to Performance PT. Without your support, we would not have enjoyed the success we’ve had over the past 18 years.
As we bid farewell to this chapter, please know our decision was not taken lightly. Several challenges have led us to this difficult choice. However, we remain committed to ensuring a smooth transition for our patients. We will work over the next few months to finalize current treatment plans through October 20. Please note Performance PT is no longer accepting new patients at this time.
Although our office may be closing, the connections we've forged and shared memories will forever remain. We thank you for being a part of our family and allowing us to be a part of your lives.
