City on a Hill Health Clinic Free medical and mental health clinic for the uninsured. We treat minor illnesses, urgent care and chronic illnesses. Services offered are:
1.

Tuesday evening general clinic for minor illnesses and limited assistance for chronic illnesses. Call main number for an appointment.
2. Wednesday morning Continued Care Clinic staffed by a an advanced practice provider. This clinic provides follow up care for those who have chronic illnesses. By appointment only.
3. Thursday morning clinic.for minor illnesses and limited assistance for chronic i

llnesses. Call main number for an appointment. Staffed by advanced practice provider.
4. Women's health clinic held every third Thursday of each month. Free pap tests, physical exams and referrals for free mammograms for those who qualify. Call the main number to make an appointment.
5. Community Health Worker consultation for assistance with housing, insurance sign up and other basic needs. Call the main number for more information.

We were truly honored to be part of West Ottawa Public Schools first-ever Asian American event. What a meaningful and beautiful space to step into.

There was such a powerful sense of pride, culture, and community—stories shared(and a very brave and joyful exchange student along with others), traditions celebrated, and opportunities created for all of us to listen, learn, and grow together.

Thank you, West Ottawa, for intentionally creating a space where diversity is not only welcomed but celebrated, and where every culture is seen, valued, and honored.

We want to take a moment to thank everyone who made today’s Sports Physical Fundraiser possible.

To our volunteer providers and nurses—your time, care, and dedication mean everything. We truly could not do this without you.

To our donors and sponsors—your generosity allows us to continue our mission of providing free mental and physical healthcare to uninsured and underinsured members of our community. You are making a real impact.
