INFECTION CONTROL IN HOSPITALS
From advanced practices in Tissue Banking to clinical applications in hospitals

Recently, the seminar on “Infection Control in Hospitals” organized by Phan Chau Trinh University of Medicine Hospital officially concluded, achieving many positive outcomes in both professional value and practical applicability.

The seminar served as a meaningful academic forum, attracting the participation of a large number of doctors, nurses, infection control specialists, and healthcare managers. The presentations not only updated new knowledge but also provided practical perspectives on applying strict infection control standards from tissue banking to clinical settings.

Expert insights highlighted the critical role of infection control in ensuring patient safety, especially in surgical procedures and tissue transplantation. Solutions related to microbiological surveillance, environmental control, and medical equipment reprocessing were presented in a clear and evidence-based manner, enhancing real-world implementation.

A key highlight of the seminar was the clear comparison between the stringent standards of tissue banks and their flexible application in operating rooms, ICUs, and clinical departments. This contributes to the development of a proactive infection control system, minimizing hospital-acquired infections and improving treatment quality.

Từ thực hành chuyên sâu tại Ngân hàng mô đến ứng dụng trong bệnh viện

Vừa qua, hội thảo chuyên đề về “Kiểm soát nhiễm khuẩn trong Bệnh viện” do Bệnh viện Đại học Y khoa Phan Châu Trinh tổ chức đã chính thức khép lại, ghi nhận nhiều kết quả tích cực cả về chuyên môn lẫn tính ứng dụng thực tiễn.

Hội thảo đã trở thành diễn đàn học thuật ý nghĩa, thu hút sự tham gia của đông đảo bác sĩ, điều dưỡng, chuyên viên kiểm soát nhiễm khuẩn và các nhà quản lý y tế. Các nội dung trình bày không chỉ cập nhật kiến thức mới mà còn mở ra nhiều góc nhìn thực tiễn trong việc áp dụng các tiêu chuẩn kiểm soát nhiễm khuẩn từ Ngân hàng mô vào môi trường lâm sàng.

Những chia sẻ từ các chuyên gia đã làm rõ vai trò then chốt của kiểm soát nhiễm khuẩn trong việc đảm bảo an toàn người bệnh, đặc biệt trong các lĩnh vực phẫu thuật và ghép mô. Các giải pháp về giám sát vi sinh, kiểm soát môi trường và xử lý thiết bị y tế được trình bày một cách trực quan, có minh chứng cụ thể, giúp nâng cao khả năng triển khai trong thực tế.

Điểm nổi bật của hội thảo là sự đối chiếu rõ ràng giữa tiêu chuẩn nghiêm ngặt tại Ngân hàng mô và khả năng áp dụng linh hoạt tại phòng mổ, ICU và các khoa phòng. Qua đó, góp phần định hướng xây dựng hệ thống kiểm soát nhiễm khuẩn chủ động, giảm thiểu nguy cơ nhiễm khuẩn bệnh viện, nâng cao chất lượng điều trị.

SURGICAL MANAGEMENT OF BOWEL OBSTRUCTION CAUSED BY DOUBLE-SITE COLON CANCER

Doctors at Phan Chau Trinh University of Medicine Hospital have successfully performed emergency surgery on a rare case of bowel obstruction caused by multifocal colon cancer—a condition that carries significant risks if not promptly diagnosed and treated.

The patient, a 55-year-old male, was admitted with progressively worsening colicky abdominal pain, accompanied by vomiting, abdominal distension, and absence of bowel movements and gas for three days. Clinical examination revealed a distended abdomen with visible bowel loops—typical signs of intestinal obstruction. Abdominal CT scan showed bowel obstruction due to a tumor at the splenic flexure, with marked dilation of the proximal colon. Additionally, another suspicious lesion was detected at the cecum.

During emergency surgery, two synchronous malignant lesions were identified in the colon: one tumor at the splenic flexure causing complete luminal obstruction—the primary cause of the condition—and a second tumor located at the cecum. This is a rare presentation and may be overlooked without thorough intraoperative exploration.

Given the complexity of the case, the surgical team decided to perform an extended right hemicolectomy, ileo-descending colon anastomosis, and ileostomy to relieve the obstruction and ensure patient safety. The procedure was successful, and the patient showed good postoperative recovery with gradual restoration of bowel function. Histopathological results later confirmed adenocarcinoma of the colon at both sites.

According to Dr. Le Van Tam, MD, Specialist Level II, multifocal colon cancer (defined as the presence of two or more tumors) is uncommon but has important implications for treatment strategy. In emergency cases of bowel obstruction, focusing solely on the primary lesion may lead to missed secondary tumors if a comprehensive evaluation is not performed. Therefore, complete intraoperative exploration of the colon or postoperative colonoscopy plays a crucial role in accurate diagnosis and definitive treatment.

In addition, the choice of surgical approach must be individualized based on factors such as the severity of obstruction, the patient’s overall condition, and the location and number of tumors, in order to optimize treatment outcomes and minimize complications.

This case highlights the complexity of bowel obstruction caused by multifocal colon cancer and underscores the importance of comprehensive diagnosis and meticulous intraoperative assessment. Timely and appropriate intervention was key to saving the patient’s life and improving treatment outcomes.

PHẪU THUẬT TẮC RUỘT DO UNG THƯ ĐẠI TRÀNG HAI VỊ TRÍ

Các bác sĩ tại Bệnh viện Đại học Y khoa Phan Châu Trinh vừa phẫu thuật cấp cứu thành công một trường hợp tắc ruột do ung thư đại tràng đa ổ – tình trạng bệnh lý hiếm gặp và tiềm ẩn nhiều nguy cơ nếu không được chẩn đoán, xử trí kịp thời.

Bệnh nhân nam, 55 tuổi, nhập viện trong tình trạng đau bụng từng cơn tăng dần, kèm theo nôn, chướng bụng và bí trung đại tiện kéo dài 3 ngày. Qua thăm khám lâm sàng, các bác sĩ ghi nhận bụng chướng, quai ruột nổi rõ, biểu hiện điển hình của tắc ruột. Kết quả chụp CT scan ổ bụng cho thấy hình ảnh tắc ruột do khối u tại đại tràng góc lách, đoạn đại tràng phía trên giãn lớn, đồng thời phát hiện thêm một tổn thương nghi ngờ u tại manh tràng.

Trong quá trình phẫu thuật cấp cứu, ghi nhận hai tổn thương ung thư xuất hiện đồng thời trên đại tràng: một khối u tại góc lách gây hẹp hoàn toàn lòng ruột, là nguyên nhân chính gây tắc; đồng thời một khối u thứ hai tại manh tràng. Đây là thể bệnh hiếm, dễ bị bỏ sót nếu không tiến hành thăm dò kỹ lưỡng trong mổ.

Trước tình huống phức tạp, các bác sĩ đã quyết định thực hiện phẫu thuật cắt đại tràng phải mở rộng, nối hồi tràng – đại tràng xuống và mở thông hồi tràng ra da nhằm giải quyết tình trạng tắc ruột và đảm bảo an toàn cho người bệnh. Ca mổ diễn ra thuận lợi, bệnh nhân hồi phục tốt sau phẫu thuật, chức năng lưu thông ruột dần được tái lập. Kết quả giải phẫu bệnh sau đó xác định ung thư biểu mô tuyến đại tràng ở cả hai vị trí.

Theo BS.CKII Lê Văn Tầm, ung thư đại tràng đa ổ (xuất hiện từ hai khối u trở lên) chiếm tỷ lệ thấp nhưng có ý nghĩa đặc biệt quan trọng trong chiến lược điều trị. Trong các trường hợp cấp cứu tắc ruột, việc tập trung xử lý tổn thương chính có thể dẫn đến nguy cơ bỏ sót tổn thương thứ hai nếu không đánh giá toàn diện. Do đó, việc thăm dò toàn bộ đại tràng trong mổ hoặc nội soi sau mổ đóng vai trò then chốt trong chẩn đoán và điều trị triệt để.

Bên cạnh đó, lựa chọn phương pháp phẫu thuật cần được cá thể hóa dựa trên nhiều yếu tố như mức độ tắc ruột, thể trạng người bệnh, cũng như vị trí và số lượng khối u nhằm tối ưu hiệu quả điều trị và hạn chế biến chứng.

Trường hợp này là minh chứng điển hình cho tính chất phức tạp của tắc ruột do ung thư đại tràng đa ổ, đồng thời khẳng định vai trò quan trọng của chẩn đoán toàn diện và thăm dò kỹ trong phẫu thuật. Đặc biệt, việc can thiệp kịp thời, đúng phương pháp đã góp phần cứu sống người bệnh và nâng cao hiệu quả điều trị.

TIMELY INTERVENTION – MULTIPLE NEWBORNS OVERCOME CRITICAL CONDITIONS

At Phan Chau Trinh University of Medicine Hospital, pediatricians have recently received and successfully treated several newborns with complex medical conditions requiring immediate intensive care after birth.
One case involved a full-term newborn at 37 weeks with intrauterine growth restriction, weighing only 1,950 grams at birth, classified as fetal malnutrition. After delivery, the infant developed multiple complications, including respiratory distress, poor feeding, vomiting, early-onset neonatal infection, coagulopathy, and polycythemia. The baby was promptly transferred to the Neonatal Intensive Care Unit (NICU) for close monitoring and aggressive treatment, including oxygen support, intravenous fluids for nutrition, intravenous antibiotics, breastfeeding support, and phototherapy. After three days of treatment, the infant’s respiratory condition improved significantly, oxygen support was discontinued, and the baby was reunited with the mother. After 10 days, the infant fully stabilized, with normal respiratory function, well-controlled infection, normalized coagulation parameters, effective breastfeeding, and was discharged.
During the same period, the hospital also admitted a woman in her third pregnancy at 33 weeks of gestation with a shortened cervix and signs of threatened preterm labor. Thanks to close monitoring and active management at the Obstetrics Department, the pregnancy was prolonged to 34 weeks and 6 days before delivery. After birth, the newborn presented with respiratory distress, polycythemia, and coagulopathy, and was immediately transferred to the NICU for monitoring and treatment. The infant received early nasal continuous positive airway pressure (NCPAP), along with intravenous fluids and phototherapy. After one day of treatment, the baby was successfully weaned off NCPAP and reunited with the mother. By day seven, coagulation indices had stabilized, the infant was feeding well, had good reflexes, and met discharge criteria.
According to MSc., Resident Physician Pham Thi Minh Thuy, early detection of risk factors during pregnancy and timely postnatal intervention play a crucial role in improving treatment outcomes and minimizing complications in newborns. These two cases highlight the effectiveness of close coordination between the Obstetrics and Pediatrics Departments, along with the hospital’s advanced neonatal intensive care capabilities.

Breast cancer remains the most common cancer worldwide and the leading cause of cancer-related death among women. According to the World Health Organization (WHO), breast cancer accounted for an estimated 670,000 deaths globally in 2022, with approximately half of the cases occurring in women without any known risk factors other than gender and age. Therefore, early detection through screening continues to play a critical role in both prevention and treatment.

In this context, Phan Chau Trinh University of Medicine Hospital organized a scientific conference titled “Updating New Trends in the Diagnosis and Treatment of Breast Cancer,” with the participation of a large number of doctors and healthcare professionals in both in-person and online formats.
