Join us for Hoops & Scoops at the Conway Public Library Park on Wednesday, August 6 from 4:30-6 pm to celebrate National Health Center Week!

There will be complimentary ice cream from the Trails End Ice Cream truck, hula hooping led by Josie Lamb, APRN, Zumba led by Billing Assistant Gisella Gambino, arthritis-friendly exercises led by Margie Riforgiato, APRN, and a houseplant and seed giveaway. Meet our providers and learn more about our programs and services!

This year's National Health Center Week celebrates the 60th anniversary of the Health Center Program, and highlights the benefits of investments in public health. Since their inception as part of President Lyndon Johnson's War on Poverty, Community Health Centers have helped people rise out of poverty, saved lives, improved public health, and reduced healthcare costs for everyone.

A delay to the funding renewal for the Health Center Program and cuts to Medicaid at the state and federal levels threaten those gains. Interim Executive Director Julie Everett Hill, RN explained, "When people are unable to afford access to medicine or basic health care, illnesses and chronic conditions that could have been managed by a routine visit to a primary care provider grow into larger issues that require more expensive hospital-based care."

NH Gives is today! Our goal this year is to raise $5000 to support our care coordination program. The federal grant that supported this program was abruptly cut off this spring. With the help of our community, we can keep it going and continue to provide this support to our patients.

Donate here: https://www.nhgives.org/organizations/white-mountain-community-health-center

Our care coordinators, Cheryl Frankowski and Erin White, work with hundreds of patients every year to help them with issues that go beyond traditional primary care but have a real effect on health and wellbeing. They help people find housing, access food, apply for disability and other programs that have really complex application processes, navigate out of abusive relationships, and so much more. You can read more about what they do on our NH Gives page above.

This approach to healthcare means our patients will have better outcomes – but it’s also just the right thing to do for people in our community who are struggling. Please help us continue this critical work!

NH Gives starts this evening! Our goal this year is to raise $5000 to support our care coordination program. The federal grant that supported this program was abruptly cut off this spring. With the help of our community, we can keep it going and continue to provide this support to our patients.

Donate here starting at 5 pm: https://www.nhgives.org/organizations/white-mountain-community-health-center

Our care coordination team helps patients when they’re facing difficult situations that affect their health and wellbeing but go beyond the scope of traditional primary care – they’ve lost their housing, they’re in an abusive relationship, they have a new diagnosis that’s really stressful and hard to navigate. Our care coordinators connect these patients with other local organizations that can help, assist with applications, help them think through next steps, and offer encouragement and support through setbacks.

This approach to healthcare means our patients will have better outcomes – but it’s also just the right thing to do for people in our community who are struggling. We are so gratified to be able to do this work, and we intend to continue despite this funding setback. But we could use your help.

We believe in a community that doesn’t leave people to struggle on their own. If you share this vision, please support our care coordination program this NH Gives Day!
