Breast Feeding…who here has struggled? Bianca required phototherapy for the first 6 days of her life, leaving me with only 20 minutes every few hours to breastfeed – not a great way to learn something that was not so easy. Once we were home, I began to regularly feed on the breast. I thought things were going great… until her pediatrician advised that Bianca wasn’t gaining enough weight. I felt defeated and disappointed in myself. I felt as though I had failed Bianca. And the sense of failure was not healthy for either of us. Thankfully, we quickly learned that a simple tongue-tie release was all that was needed to help with feeding. I am grateful that Bianca now feeds well from the breast, from the bottle, and likes formula. She has options!!

Maternal mental health is not discussed enough. In honour of world Maternal Mental Health Day, I raise a glass to all moms out there!! Whatever it is that might make you feel defeated, remind yourself that you are doing your best, you are learning everyday, and that you are doing what is best for you and your baby!!

This week was infertility awareness week. Did you know 1 in 6 people have difficulties conceiving. Everyone scrolling on here likely knows someone who has had problems or someone who might be suffering silently. Culturally it has become accepted for people to ask women of reproductive age if they will be having kids, or if they will have a second or third, or when are they having kids. These questions may be well intentioned, but they may cause someone pain and heart ache. To all women and men struggling, or have struggled, sending lots of love, hope, courage and strength as you embrace this journey.

Painful periods (dysmenorrhea) is sadly common and widely accepted as normal in our culture, especially amongst adolescents. Women will often miss school, work, and/or social activities due to pain. NSAIDS are often the first line of defense for pain management. This is a great systematic review (scroll to the left to see article details, full article can be found on PubMed) showing that exercise, specifically low intensity exercise like stretching or yoga and heat therapy can be an effective alternative to over the counter analgesics. Acupressure also proved to have beneficial effects for these women. So what does this mean? Lifestyle changes and self-care for the win!!

Please remember, painful periods should not be ignored. Always speak to your health-care team so they can help find the root cause. There are many things one must consider. Who is in your web of support to help with your painful periods?

Ending 2020 with a sense of gratitude. Although this year challenged our way of living, it is fair to say we all have learned a thing or two. Nearly three years ago, I opened Advanced Pelvic Physiotherapy Clinic with two amazing women . A multidisciplinary centre for men and women that specializes in pelvic floor function. We are fortunate to collaborate with some of Toronto’s best Gynecologists, Urologists, Urogynecologists, Colorectal Surgeons, Gastroenterologists, Family Doctors, Paediatricians, Midwives, Doulas and many more amazing health care providers. Grateful is an understatement. What 2020 has taught me is that having a calendar packed with professional and social obligations isn’t necessary to feel satisfied in my professional and personal life. Providing evidence-based therapy and helping people everyday is incredibly rewarding. Sometimes when we are moving too fast, we forget our purpose. I want to thank all my family, friends, clients and referral sources for your continued support over the years (entering into 13 years of practice). I wish everyone a safe, healthy and happy new year. Entering into 2021, I will continue to do my best at setting time aside to empower you all through this platform with education and mindful thoughts.

I see a lot of women and men who suffer with body image issues, poor self-compassion and self-love. All of this can cause a plethora of health issues including eating disorders. Many of my patients struggle with pelvic pain, which can be driven by many things. If you have had an eating disorder you know well that sometimes the site of food can cause negative thoughts and/or fears. These thoughts can cause physiologic changes in our system, including the flow of digestive juices, saliva and enzyme productions, and changes in digestive hormones. Essentially how our nervous system interprets food can influence how we digest food. Getting a thorough history from my patients is so important. Pelvic pain can be complicated, but when we listen to the person in front of us and help them unpeel their onion we can work on rewiring their nervous system.

Did you know food can help fertility? Specifically, a Mediterranean Diet has been shown to improve fertility. It is associated with a 69% lower risk of ovulatory disorder in women with infertility and a 40 % increased success rate with IVF (Santangelo C., et al. 2014). If you are entering preconception or have been trying to get pregnant perhaps consider talking to your health care providers about how you can change your diet to optimize your fertility success. Don’t give up, look at all avenues available💕

Happy Friday!! Not sure about you, but I feel like the weeks are flying by. Does that mean I am doing too much and not stopping to get in enough self care, perhaps? I try to spend 5 minutes a day, at minimum, doing intentional breathing. This week there were a few days I missed my minimum 5 minutes of quiet and I am feeling it today.

Did you know deep breathing not only evokes a relaxation response, but it can also help soften your pelvic floor? When you take a slow deep breath in your diaphragm and pelvic floor both descend.

Inhale for the count of 4 letting your ribs and belly expand and exhale for the count of 5 through pursed lips- it feels so nice and your pelvic floor will thank you. We really underestimate the power of breathing. Also, don’t forget, when breathing in breath in calm thoughts and when breathing out breath out tension. Have a great weekend everyone. Xo

Did you know that 60% of perimenopausal women report brain fog and memory loss? 😳

There are a number of reasons why this occurs. Here are few reasons...

- Decrease in natural estrogen
- Women with increased cortisol levels are more often to complain about hot flashes, depressive or anxiety symptoms and decline in cognitive performances.
- Diets high in carbohydrates can cause dysglycemia ( abnormality in blood glucose). Dysglycemia can cause cognitive impairments in perimenopausal women.

Perimenopausal women with mild cognitive impairment, episodic memory loss without dementia, are more likely to progress to dementia (including Alzheimer’s disease). This is scary, but lifestyle modifications can help!!!

- Reduce xenoestrogen (foreign estrogen) exposures, and speak to your health care team how to create a smooth transition for your hormones
- Learn how to cope with stress
- Control blood sugar
- Reduce inflammation, control body weight and body fat

Many of my patients are on Oral Contraceptives (OCPs) for many reasons other than the obvious, a form of birth control. Some love being on them and would not want it any other way while others struggle with the idea and don’t like how they make them feel. Like anything else we need to be informed and weigh the pro’s and con’s. Some research has shown OCPs can increase the risk of breast and cervical cancer whereas risks of endometrial, ovarian and colorectal cancers are reduced. The other thing one needs to consider is some micronutrients are not well absorbed when on OCPs.

• Folic Acid
• Vitamins B2,B6, B12
• Vitamins C and E
• Magnesium, selenium and zinc

Questions to ask yourself, family members and primary care provider / gynecologist when choosing this method of birth control: do any of these cancers run in your family? What are my other options? Is it helping me? Do I have a healthy diet and should I be talking to my doctor or allied health care provider about nutrient repletion.
