For over 33 years we have partnered with families caring for an aging parent.

Aging Well Rochester, a Division of Reflections Management & Care, provides compassionate care management, advocacy, dementia support, PRI/screens, and in-home care services helping older adults age with dignity and support. Our goal is to listen to your concerns and use our professional skills and knowledge of community resources to create a custom plan for care and support that meets your parent

s' needs. Each member of our Care Management Team is a licensed Masters level social worker and/or registered nurse. We have expertise working with medically complex older adults and those suffering from memory loss.

* Coordinate and supervise in-home services

* Placement in senior residences, assisted living and nursing homes
