They develop from the same embryologic tissue. That means they share:

✔️Erectile tissue
✔️A g***s (head)
✔️A shaft
✔️Crura (internal “legs”)
✔️A hood/foreskin
✔️A rich nerve supply

Curious what the difference is? Mostly size, external visibility, and hormonal influence during development.

Meet your Big O Orchestrators!

These pelvic floor muscles (especially the bulbospongiosus & ischiocavernosus that wrap around your erectile tissue) play a key role in pleasure physiology.

Here’s the sequence most people don’t realize:

✨ They must start relaxed → so blood can fully fill the cl****al erectile tissue and vestibular bulbs.
✨ Then they contract strongly and rhythmically → helping trap blood in these areas and to create the pulsations of or**sm.

No relaxation? Blood flow is limited.
No strength or full range? Or**sm intensity may suffer.

It’s not just “tighten and squeeze.”
It’s relax → fill → contract → sustain.

Research shows:

Stronger pelvic floor function correlates with higher arousal and satisfaction scores (Lara et al., 2014).

EMG studies demonstrate rhythmic pelvic floor contractions during Or**sm (Levin, 2009).

Your pelvic floor is not just about continence or core stability.

It’s part of your pleasure anatomy.

But in all seriousness…

Your annual gynecologic exam should NEVER hurt.

Pressure? Maybe.

Mild sensitivity? Possibly.

Sharp pain, burning, bracing, or tears in your eyes? Not normal.

If your exam hurts, that is clinical information, not something to “just endure & tolerate.”

Pain can be related to:

👉Pelvic floor muscle hypertonicity
👉Vulvar tissue or hormonal changes
👉Nerve sensitivity
👉Inflammation or dermatologic conditions
👉Prior trauma patterns

And yes… it is treatable.

Cl****al pain, clinically referred to as clitorodynia, is one of the most complex and nuanced conditions we treat in pelvic health.

“Normal” exams do not rule out dysfunction.

When we differentially diagnose cl****al pain, we evaluate multiple overlapping systems, including:

👉Nerve Driven
Pudendal (specifically The Dorsal/Cl****al branch) Genitofemoral and/or Ilioinguinal nerve irritation

👉PFM Driven
Pelvic Floor Muscle tension - 2 PFMs attach on the cl****al hood

👉Hormone Driven
Low estrogen and testosterone → thinner, sensitive tissue

👉Infection / Inflammation
Yeast, bacteria, dermatitis, or infection under the cl****al hood

👉Structural / Friction
Adhesions, keratin pearls, tight clothing, cycling

👉Sexual / Arousal Factors
PGAD, post-or**smic pain, inadequate lubrication, overstimulation

👉Referred Pain
Bladder, endometriosis, hip, or spine sources (Tarlov cysts)

👉Nervous System Driven
Trauma, central sensitization

Take-home message...

It is rarely “just one thing.” For most women, this is multifactorial: a combination of tissue, nerve, hormonal, and nervous system influences.

And yes, finding clinicians trained to properly evaluate and treat this can be challenging.
