Massage therapy has changed a lot over the last decade.

Many therapists were originally trained in one of two ways:

• Spa / relaxation massage
• Deep tissue / sports massage

Both approaches can have value.

But modern research is helping us understand how and why manual therapy works in a much more nuanced way.

Today’s soft tissue therapists don’t just follow routines or rely on pressure.

They assess.
They reason clinically.
They adapt their treatment to the individual in front of them.

That’s the approach I teach on my Level 4 Soft Tissue Therapy Diploma.

As a Level 4 Soft Tissue Therapist, our role isn’t to diagnose — it’s to assess, reason, and decide on an appropriate course of action within our scope.

This presentation doesn’t scream “tear” or “strain.”
It suggests a possible mismatch between load and tissue capacity.

Palpation tenderness alone does not equal injury.

The real skill isn’t knowing lots of techniques.
It’s knowing when and why to use them.

This is the difference between Level 3 thinking and Level 4 clinical reasoning.

In a case like this, soft tissue techniques are supportive — not corrective.

The aim isn’t to “fix a tight muscle.”

It’s to modulate symptoms and improve load tolerance while we address training variables.

This is the difference between technique-led practice and reasoning-led practice.

If you’re interested in supporting people in pain, after injury, stress, or persistent discomfort, you’ve probably heard a lot about trigger points.

Here’s how I teach them on our Level 4 Soft Tissue Therapy Diploma 👇

✅ What trigger point / sustained pressure work is
• A manual technique involving sustained, tolerable pressure
• Used to modulate pain and sensitivity
• Often helpful as part of a broader treatment plan
• A way to support movement, confidence, and comfort

❌ What it isn’t
• It doesn’t “release knots”
• It doesn’t break down adhesions
• It doesn’t permanently change muscle structure
• It isn’t a standalone fix for pain

🎯 Primary effects
• Short-term pain reduction
• Reduced local tenderness
• Increased tolerance to movement
• Reassurance for people who feel “tight” or sore

🧠 Associated mechanisms (what’s most likely happening)
• Nervous system–mediated pain modulation
• Activation of descending pain inhibitory pathways

• Temporary changes in sensitivity, not tissue damage repair

🩺 Clinical reasons a therapist might use it
• The client reports localised tenderness
• Pressure is consensual and tolerable
• Pain or guarding is limiting movement
• It supports exercise, rehab, or self-management
• The goal is symptom modulation, not “fixing tissue”

🚫 Outdated narratives we don’t teach
• “Trigger points are toxins”
• “You must eliminate them for pain to resolve”
• “Pain equals damage”
• “More pressure = better results”

Not Quite Ready for Level 4? Start with Level 3 Standards – Then Upgrade Later

Not everyone joins our Level 4 Soft Tissue Therapy Diploma ready to work in an injury or remedial setting straight away and that’s absolutely fine.

Some students want to start by working with healthy individuals, build confidence, gain experience, and step into the industry at a pace that feels right for them.

That’s why we offer a Level 3 Standards Pathway within the Level 4 Diploma.

🔹 This is not a separate qualification
Instead, you complete the Level 3–aligned components of the Level 4 course, with the option to upgrade later.

What the Level 3 Standards Pathway Includes
✔ Attend 10 of the 16 training dates
✔ Complete selected Level 3–aligned assessments
✔ Receive a Level 3 Standards Achievement Certificate
✔ Gain Associate Membership with the Sports Therapy Association
✔ Begin working as a non-remedial soft tissue therapist

You’ll be able to work with healthy individuals only, offering treatments in the context of:
• Pre-event
• Post-event
• Maintenance
• Injury prevention

Pain isn’t always coming from damaged tissue.
And that matters more than ever for soft tissue therapists.

Central sensitisation is when the nervous system becomes overprotective, amplifying pain even when tissues are healing or structurally sound.

👉 For a soft tissue therapist, this changes everything:
• Pain ≠ tissue damage
• More pressure ≠ better results
• Assessment, communication and reassurance become treatment tools
• Touch can calm the nervous system — or provoke it

Clients with persistent pain may present with:
• Pain that feels disproportionate to findings
• Widespread or fluctuating symptoms
• Heightened sensitivity to pressure
• Fear, uncertainty or frustration around movement

This is why modern soft tissue therapy must go beyond techniques alone.

One of the most common explanations given for massage is that it “increases blood flow” or “flushes toxins from the muscles.”

You’ll often hear people say things like “it brings fresh blood into the area” or point to red skin as proof that healing is happening.

But how accurate are these explanations?

Modern research suggests the answer is more nuanced, and far more interesting than the traditional mechanical explanations many of us were taught.
