Let’s take a look at the latest updates in early triple negative breast cancer (TNBC) treatment 💜

Here’s what the latest trials are showing👇

1️⃣ KEYNOTE-522

The KEYNOTE-522 trial established pembrolizumab (Keytruda) plus chemotherapy before surgery, followed by pembrolizumab after surgery, as the current standard of care for many patients with early TNBC.

This approach reduced the risk of the cancer coming back compared to chemotherapy alone.

2️⃣ NeoSTAR

The NeoSTAR trial is exploring whether sacituzumab govitecan (Trodelvy, an ADC) combined with pembrolizumab before surgery could be an effective alternative to standard chemotherapy.

Early results show promising response rates.

3️⃣ SASCIA

The SASCIA trial is testing sacituzumab govitecan after surgery in patients who still have cancer remaining after chemotherapy.

This could potentially replace standard follow-up chemotherapy if results are positive.

4️⃣ ASCENT-05 / OptimICE-RD

This trial is evaluating whether adding sacituzumab govitecan to immunotherapy after surgery can improve outcomes for patients with residual disease.

Results are still ongoing.

5️⃣ TROPION-Breast03

This trial is testing datopotamab deruxtecan (Datroway, an ADC) with or without immunotherapy compared to standard treatment.

It may offer another targeted option in early TNBC.

6️⃣ GeparNuevo

The GeparNuevo trial showed that adding durvalumab (immunotherapy) to chemotherapy improved long-term outcomes, suggesting more immunotherapy options may become available.

7️⃣ Vaccines

Researchers are also exploring vaccines that train the immune system to recognise and attack TNBC cells, with several trials underway.

Metastatic Monday: Understanding HR+ metastatic breast cancer 💜

HR+ HER2- is the most common subtype of metastatic breast cancer, but there is still a lot within that diagnosis that can influence treatment options 💊

Your exact HER2 status matters more than ever. Newer categories such as HER2-low and HER2-ultralow may open the door to additional treatments for some people 📊

Your care team may also test for gene mutations such as PIK3CA, ESR1 or BRCA. These results can help guide whether certain targeted therapies may be suitable for you 🧬

In the UK, first-line treatment for HR+ HER2- metastatic breast cancer often combines hormone therapy with a CDK4/6 inhibitor, such as ribociclib or abemaciclib.

If a treatment stops working, your team may move on to the next line of therapy. Treatment options are continuing to grow, with newer targeted drugs available on the NHS and some antibody-drug conjugates available through private care 👩‍⚕️

Understanding your subtype, biomarker testing and treatment pathway can help you feel more informed when discussing your options with your care team 💜

Breast cancer breakdown: Mastectomy💜

A mastectomy is a surgical procedure that involves removing the entire breast tissue👩‍⚕️ Here are the key types⬇️

🔸Total (simple) mastectomy: removes the whole breast, including ni**le and ar**la. Usually does not remove underarm lymph nodes or chest muscles.

🔸Skin-sparing mastectomy: removes breast tissue and ni**le/ar**la but keeps most of the breast skin. Often done if immediate reconstruction is planned.

🔸Nipple-sparing mastectomy: removes breast tissue but keeps the skin, ni**le, and ar**la. Suitable only for some people.

🔸Modified radical mastectomy: removes the whole breast and underarm (axillary) lymph nodes, but chest muscles are kept.

🔸Radical mastectomy: removes the whole breast, underarm lymph nodes, and chest muscles. Rarely done today.

🔸Partial (segmental) mastectomy: removes the cancer and a larger area of surrounding tissue, but not the whole breast.

🔸Keep a simple routine - do harder tasks when you feel most alert, avoid multitasking and prioritise sleep/rest⏰

🔸Cognitive rehab can help - brain training (crosswords/ sudoku) and some people explore CBT/MAAT or EEG biofeedback (ask what's available locally)🧩🔢

Breast Cancer Breakdown: Breast Conserving Surgery💜

Breast conserving surgery (BCS) allows cancer cells to be removed while maintaining a normal breast shape and appearance🧫

This procedure removes cancer, as well as a rim of healthy tissue that surrounds it. The sphere of tissue removed around the tumour is known as the “surgical margins” of cancer🔘

It is important that your surgical margins come up as “negative” or “clear” to make sure that all the cancer cells have been removed❌

Radiation will often follow to reduce the chance of cancer returning to the site at a later date☢️

Some people may need a second operation if margins aren't clear👩‍⚕️

BCS is not always possible - it depends on tumor size, location, and breast size🙏
