โฌ› To understand this condition, it is helpful to look at the anatomy.

โฌ› The iliopsoas is a powerful, biarticular hip flexor that stabilizes the trunk, maintains posture, and enables running.

โฌ› It is comprised of three parts: the psoas major, the psoas minor (which is sometimes absent), and the iliacus muscle.

โฌ› The iliacus muscle fans out across the pelvis and eventually joins the psoas major at the root of the thigh.

โฌ› Within the belly of the iliacus muscle, there are two distinct groups of muscle bundles: the medial fibers and the lateral fibers.

โฌ› These two sections are separated by a reinforcing connective tissue plate known as the "intramuscular fascia of the iliacus muscle" (IMFI).

โฌ› This fascia dives deep inside the muscle, joining the epimysium to almost completely cover the medial belly of the iliacus.

โฌ› In this newly described syndrome, patients experience severe inguinal painโ€”especially during hip flexion and extensionโ€”due to an isolated structural alteration within this specific fascia.

โฌ› Chronic overuse and fatigue of the iliopsoas muscle can trigger inflammation and significant thickening of the IMFI.

โฌ› Because this fascia acts like a thick, semi-circumferential sleeve surrounding the medial fibers of the iliacus, its thickening essentially confines the muscle bundles.

โฌ› This confinement leads to pronounced myofascial stiffness, muscular contracture, and chronic pain that can be present even at rest while sitting.

โฌ› Historically, diagnosing iliopsoas issues has been challenging because the muscle sits deep within the pelvis and can mimic other skeletal, visceral, or pelvic issues.

โฌ› An accurate, high-frequency ultrasound (US) evaluation is highly effective for diagnosing this specific fascial thickening.

โฌ› When examined via ultrasound, patients with this syndrome showed a marked thickening of the IMFI on their painful side, averaging 2.70 mm in thickness compared to just 1.02 mm on their healthy side.

โฌ› Furthermore, strain elastosonography (ELS) revealed a clear increase in muscular stiffness in the medial iliacus, while power Doppler sometimes detected increased perifascial vascular signals, indicating localized inflammation.

โฌ› Chronic hip and groin pain lasting over three months.

โฌ› Pain that worsens during thigh flexion-extension movements and running.

โฌ› A positive Thomas test, which indicates a shortened and contracted state of the hip flexors.

โฌ› Preserved range of motion (ROM) and negative hip impingement tests (like FADDIR), which helps rule out joint osteoarthritis.

โฌ› Importantly, ultrasound and subsequent MRI imaging allow physicians to easily rule out other common athletic injuries, such as hernias, bursitis, tendon avulsions, or structural muscle tears.

โฌ› The most promising aspect of this discovery is that the condition responds excellently to non-invasive treatments.

โฌ› All patients in the documented case series successfully recovered through conservative physical therapy.

โฌ› The rehabilitation protocols focused on:

โฌ› High-energy laser therapy to target and reduce inflammation in the involved myofascial region.

โฌ› Dedicated stretching and relaxation exercises targeting the thigh flexors to relieve the myofascial contracture and restore muscle function.

โฌ› Through these conservative methods, patients experienced a slow, gradual resolution of their pain syndrome.

โฌ› Only in rare instances were non-steroidal anti-inflammatory drugs (NSAIDs) additionally required to achieve significant clinical improvement.
