Infertility is a scary word — but you’re not alone. ♥️

June is World Infertility Awareness Month, and at CSD, we’re opening up the conversation around male infertility and the important role s***m donation can play in helping families grow.

What is infertility?
Infertility is a reproductive condition where couples cannot conceive, while actively trying, for more than a year. It affects 1 in 6 people, and yes, it impacts men and women equally. Where 50% of infertility cases are as a result of male infertility and 50% to female infertility (World Health Organization; 2023).

But still, male infertility often gets overlooked.

What causes male infertility?
There isn’t one simple answer. It’s different for everyone. Some common causes include:

🧬Age, genetics, hormone imbalances, and lifestyle factors
💊Certain medications or health conditions,
❓And sometimes, there is no clear answer at all.

And yet, despite this, the burden of infertility is still too often placed on women.

When should you seek infertility assistance?
If you have been trying to conceive for over a year without success, it’s worth speaking to a professional. Even for healthy couples aged 29-33, there is only a 20-25% chance of conceiving per month, this increases to 60% over 6 months (Centers for Disease Control and Prevention, 2024).

How is infertility treated?
The good news is that most cases (85–90%) can be treated with medication or straightforward procedures.
Only a small percentage (under 3%) require more advanced treatment like IVF (xx).

✨1 in 7 babies today are born through fertility treatments.
✨Only 6% of infertility cases in men are a sign of something more serious.
✨IVF success rates using donor s***m are approximately 51% per cycle, offering a hopeful path to fatherhood for many men facing fertility challenges
(American Journal of Obstetrics and Gynecology, 2017).

✅ FACT: In South Africa, the law limits each s***m donor to a maximum of twelve live births. 🚫👶

This regulation is in place to protect donor-conceived families and reduce the chance of any genetic overlap.

Fun fact: South Africa has a unique ancestral gene pool! 🇿🇦 We analyse our s***m donors’ ancestral heritage closely when they donate:

Black African of Southern African descent (an important note for this group is there is a low incidence of sickle cell disease (SCD)).
East Indian.

Our European population can trace their ancestry to Dutch, French, Portuguese, German, Greek, Norwegian origin.

Our mixed origin donors have a racial mix of European, black African, and Asian origin.

How is s***m created? The process of s***matogenesis! 🔬

🧪 Starting Point: Germ Cells
Inside the seminiferous tubules, tiny structures in the testicles, are special cells called s***matogonia — these are the s***m "stem cells." They go through several stages to become mature s***m.

🔄 Cell Division: Meiosis
S***matogonia first divide by mitosis(to keep making more of themselves), and then some of them enter meiosis, a special kind of cell division that cuts their chromosome number in half. This is important because s***m carry only half of the DNA (23 chromosomes), which will combine with the egg’s 23 chromosomes to make a full set (46).

🧬 Shaping Up: S***miogenesis
After meiosis, the resulting cells (called s***matids) aren’t quite ready yet — they still need to mature into s***m. This process is called s***miogenesis:
- They grow tails (flagella) for swimming.
- The head forms, containing the DNA and an enzyme cap called the acrosome to help pe*****te the egg.
- Excess cytoplasm is discarded.

🚚 Transport & Final Maturation
Once they're fully formed, s***m move to the epididymis (a coiled tube behind the testicle) where they mature further and gain the ability to swim. They can be stored there for several weeks.

⏱️ Timeline
The whole process takes about 64 to 72 days from start to finish, and it’s constantly ongoing after puberty.

Yes! Your identity as a s***m donor will be kept completely confidential and you will only be known by your donor number. 🤫 This is ensured by South African legislation.

The only details recipients receive relate to some relevant physical attributes (height, hair colour, etc), interests and educational background. This will help them choose a donor that matches their looks and values.

Having the opportunity to become a parent and raise a child is–on paper–a right that belongs to everyone, and yet when the topic of fertility treatment arises there is a certain sense of shame that many people exploring this route come to feel.

Whether the heterosexual couple suffering from PCOS, or the single woman looking to become a mom, or the same-sex couple wanting to start a family, it is very normal for this feeling of embarrassment to creep in.

It could be argued that the best way to dispel these stigmas is by normalising the need and right to fertility treatment, and by having a conversation on all the permutations that a person or couple seeking the treatment might take. Some stigmas we’ve detected in society include:

Women who are unable to successfully conceive and carry a healthy baby
Men who are unable to father children
Women looking to become single mothers
Same-sex couples wanting children

These are all perfectly normal and acceptable scenarios, in which all parties have the right to a shot at starting a family, and that needs to be known.
