✨ Patient Journey | Liezel ✨

Liezel has been living with chronic lower back pain since 2018. A year ago, she underwent an amputation of her right leg. As part of her recovery, her left hip, knee and ankle had to adapt to new physical demands, which contributed to ongoing discomfort. She also experienced phantom limb sensations on her right side.

After multiple surgeries and extended hospital stays, Liezel was referred to our Panorama Pain Clinic for further assessment and support.

Working alongside our multidisciplinary team developed an individualised management and rehabilitation plan tailored to her specific needs. Her treatment plan included interventional pain procedures and a structured rehabilitation programme, including hydrotherapy, aimed at improving strength and function in preparation for prosthetic fitting.

Liezel has since progressed to the next phase of her rehabilitation, including receiving her prosthesis, and continues to work closely with her healthcare team as she moves forward in her recovery journey.

Every patient’s condition and response to treatment is unique, and management plans are tailored accordingly.

Term of the Day: Radiculopathy

Synonyms:
Nerve root compression, nerve root irritation, pinched nerve, sciatica (when affecting the leg), cervical radiculopathy (neck and arm symptoms), lumbar radiculopathy (back and leg symptoms)

What it means (plain English):
Radiculopathy occurs when a spinal nerve root — the part of the nerve that exits the spine — becomes compressed or irritated. This may be caused by a disc herniation, degenerative changes, or narrowing of the spinal canal (stenosis).

Unlike mechanical back pain, which stays in one place, radiculopathy may cause radiating pain, numbness, tingling, or weakness that travels along the path of the affected nerve. In the lower back, this often involves pain down the leg (sciatica). In the neck, it may cause symptoms in the shoulder, arm, or hand.

Natural history:

• Many cases improve over time as inflammation settles and the body adapts.

• When a disc presses on a nerve, symptoms may reduce over weeks to months as the body gradually reabsorbs disc material.

• Some cases persist longer, but serious or permanent nerve damage is uncommon.

• Early rehabilitation may help support recovery and reduce long-term nerve sensitivity.

Concepts of management:
Treatment aims to reduce nerve irritation, relieve pressure, and restore movement. Approaches may include:

• Staying active within tolerance to prevent stiffness

• Medication such as anti-inflammatories or short-term nerve-calming medication

• Physiotherapy with targeted exercises and nerve mobilisation techniques

• Epidural steroid injections for severe or persistent symptoms

• Surgery is considered for progressive weakness, bladder or bowel symptoms, or pain that does not improve with comprehensive conservative care

Radiculopathy can often be understood as a temporary disruption in nerve signalling rather than permanent damage. With appropriate care and guidance, many individuals experience meaningful improvement in symptoms and function.
