‘Restoring form and function’

The pre-operative images demonstrate post traumatic dorsal saddling with and an under-rotated bulbous nasal tip together with nasal axis deviation.
Septal reconstruction to correct a weak and buckled nasal septum is key to restore nasal function and aesthetics.

Technical details included a 3mm dorsal reduction, bilateral asymmetric oblique and lateral osteotomies, septoplasty with perpendicular plate splinting of the caudal L-strut, repositioning of the caudal septum, bilateral extended spreader grafts, septal extension graft and tip refinement with lateral crural transposition with lateral crural strut grafts.

Complex Revision Surgery: on-table before and after:

These pictures show the result from a recent tertiary, structural septorhinoplasty. The patient presented with severe nasal and septal deviation with airway obstruction. Surgery focuses on both the aesthetic and functional improvements. Technical details included:
- Caudal septum replacement graft for a straight, ‘functional’ septum.
- Nasal bones straightened with nasal fractures and osteoplasty using the piezotome.
- Bilateral asymmetric extended spreader grafts to stabilize the caudal septal replacement graft and reconstruct the mid-vault. - Nasal tip projection, correction of rotation and alar retraction: turn-under flaps, lateral crural steal and articulated rim graft.
- Rib cartilage was required for the reconstruction as the septal cartilage was both deficient and deformed. Revision rhinoplasty’s are complex and challenging and require experience and specific skills. Choose your surgeon wisely.
