George Surgical Centre would like to congratulate Garden Route Hand Surgeon, Dr. PC Pelser, on reaching a remarkable milestone in his surgical career by performing his 100th CMC joint replacement!

What is a CMC joint replacement?

A CMC joint replacement is a surgical procedure where the arthritic joint at the base of the thumb, called the carpometacarpal (CMC) joint, is removed and replaced with an artificial implant, typically done to alleviate pain and improve function caused by arthritis in that area; essentially, it's a thumb joint replacement surgery.

A successful CMC joint replacement can significantly improve thumb function, reduce pain, and allow for better hand movement.

The Garden Route Hand Unit, based in Gloucester Lane, George, specializes in the management of all diseases and injuries affecting the hand, elbow and peripheral nerves.

The Essential Role of the Embryologist in IVF

This week we turn our focus to our embryologist, Lizelle Griessel, contributions to our IVF program.

Should your IVF journey come to the point where the decision is made to proceed, you will get to know Lizelle very well during the various stages of your IVF journey. This takes place at the George Surgical Centre where our fully accredited laboratory is located.

The IVF process entails the following steps:
• Egg Retrieval and Assessment:
After you have been stimulated to produce multiple follicles and our team has retrieved your eggs from these follicles, Lizelle meticulously identifies and collects them from the follicular fluid. She then prepares the eggs for fertilization.
• S***m Preparation:
She would have performed a detailed semen analysis before the IVF to evaluate s***m count, motility, and morphology. On the day the eggs are retrieved she processes the semen to isolate the healthiest and most motile s***m for fertilization.
• Fertilization Techniques:
Eggs and s***m are combined in a culture dish and incubated to allow them to fertilize naturally. The fertilized eggs are closely monitored as they develop into embryos.
Lizelle maintains optimal cultural conditions to support their growth to ensure they are developing normally.
• Embryo Grading and Selection:
Lizelle uses specific grading criteria to evaluate the embryos based on cell number, symmetry, fragmentation, and morphology. This grading helps select the best-quality embryos for transfer or cryopreservation.
• Embryo Transfer Preparation:
Working closely with our team, Lizelle selects the highest-graded embryos that will be carefully loaded into a transfer catheter and are typically transferred after 5 days. Surplus high-quality embryos are cryopreserved using specialized vitrification techniques for future use.
• Cryopreservation and Thawing:
Lizelle performs vitrification to freeze embryos, ensuring their viability for future IVF cycles. When the time comes for another pregnancy, she will carefully thaw cryopreserved embryos when needed for transfer.
• Ensuring Quality and Success:

Lizelle is responsible for maintaining the highest standards of quality control in our accredited laboratory. Her attention to detail and expertise have had a tremendous impact on the high success rates of our IVF treatments. She is known for her professionalism and dedication. She provides personalized care to each patient, ensuring they understand the process and feel comfortable throughout their journey. Her compassion and expertise have earned her the respect of both her team and patients.
