We are liaising and working together with all the other health disciplines like(Hospitals, Stepdown facilities and Rehab centres, Oncology centres, Doctors, Pharmacists, Physiotherapists, Speech therapists Social workers, Wound Care specialists, etc.).. My Care! We Care.
