Fat transfer has evolved dramatically in recent years, transforming from an unpredictable procedure to a refined technique with consistent results. The key advancements that have improved outcomes include:

• Gentler harvesting techniques that preserve fat cell viability
• Processing methods that purify the fat while maintaining stem cell content
• Strategic layering of micro-droplets rather than large boluses
• Better understanding of overcorrection requirements (typically 20-30%)
• Improved post-procedure protocols to optimise graft survival

When performed with these advanced techniques, approximately 60-80% of transferred fat typically remains permanently, with results visible after initial swelling subsides (usually 4-6 weeks).

The procedure offers significant advantages: using your own tissue eliminates rejection risk, results look and feel natural, and the harvested fat contains stem cells that may improve surrounding skin quality.

Fat transfer does require expertise—it's truly where surgical technique meets artistic sensibility—but in experienced hands, it provides predictable, beautiful volume restoration.

Ear prominence might seem like a minor concern to some, but for those affected, it can significantly impact confidence and daily life. This common genetic trait typically results from underdeveloped antihelical folds (the inner curve of the ear cartilage) or an overdeveloped co**ha (the bowl-shaped cavity).

Otoplasty offers a relatively straightforward solution with high satisfaction rates. The procedure addresses these concerns by creating or enhancing the missing folds and/or reducing the co**hal cartilage. This outpatient procedure takes approximately 1-2 hours under local anesthesia for adults or general anesthesia for children.

Recovery involves wearing a protective headband continuously for one week, then nightly for 5-6 weeks to protect the ears during healing. Most swelling resolves within 2-3 weeks, though complete resolution can take several months.

For children, the procedure is typically performed after age 5-6 when ear growth is nearly complete and before potential social impacts affect self-esteem.

Otoplasty can be performed for both children and adults, with customised approaches based on each patient's specific ear anatomy.

What drew me to plastic surgery? The answer lies in its unique blend of precision and creativity—it's both exacting science and expressive art.

Unlike many surgical specialities focused solely on function, plastic surgery demands aesthetic judgment alongside technical expertise. Whether reconstructing facial features after trauma or refining nasal contours, each case presents a three-dimensional puzzle requiring both surgical skill and artistic sensibility.

What's most fulfilling is witnessing the profound psychological impact of our work. When a breast reconstruction patient sees herself whole again, or when someone's outward appearance finally aligns with their self-image—these moments remind me why this speciality continues to inspire me daily.

The privilege of helping patients rediscover confidence or restore function after injury continues to make plastic surgery an immensely rewarding field, even after all these years of practice.

Tummy tuck procedures can be customised to address specific anatomical concerns. The appropriate technique depends on your unique anatomy, degree of skin laxity, muscle separation, and aesthetic goals.

• Full abdominoplasty addresses concerns from p***c bone to ribcage. Includes muscle tightening (especially important after pregnancy), removal of excess skin and fat, and often belly button repositioning. Creates the most dramatic improvement for significant skin laxity and muscle separation.

• Mini-abdominoplasty focuses on lower abdomen only (below navel). Uses shorter incision and typically doesn't involve navel repositioning. Ideal for patients with concerns limited to lower abdomen with minimal skin laxity and good muscle tone.

Extended abdominoplasty is an expanded version of full abdominoplasty with incision extending around flanks. Addresses excess tissue on sides and lower back in addition to abdominal concerns. Appropriate for patients with circumferential excess tissue, often following significant weight loss.

Fleur-de-lis abdominoplasty includes vertical midline incision in addition to horizontal incision. Provides maximum skin removal both vertically and horizontally. Reserved for cases with significant horizontal and vertical excess, typically after massive weight loss.

This frequent question highlights an important distinction in breast enhancement procedures:

Breast augmentation (augmentation mammoplasty) focuses on increasing breast size or volume using silicone or saline implants. It's ideal for patients seeking larger breasts or to restore volume lost after pregnancy or weight loss, but doesn't significantly change breast position.

Breast lift (mastopexy) addresses breast sagging (ptosis) by removing excess skin and reshaping tissue to raise the breast position and ni**le height. This procedure improves breast shape and position but doesn't substantially change size.

Many women benefit from combining both procedures (augmentation-mastopexy) to address both size and position simultaneously. The right approach depends on:

• Your current breast tissue amount
• The degree of sagging present
• Your desired final breast size
• Ni**le position relative to the breast crease

During consultation, I will evaluate these factors and discuss which procedure—or combination—would best achieve your aesthetic goals.

Recovery from abdominoplasty (tummy tuck) happens in stages, and understanding this timeline helps patients plan accordingly:

First 1-2 weeks: This is the most intensive recovery period. You'll need assistance at home, especially if you have young children. Walking bent over is common due to tightness, gradually improving to upright posture.

Weeks 2-4: Most patients return to desk jobs and light daily activities. Fatigue remains common, and wearing a compression garment is important.

Weeks 4-6: Exercise can gradually resume, starting with lower-intensity activities and avoiding core-focused workouts.

Weeks 6-8: Most restrictions lift, though complete internal healing and final results continue developing for up to a year.

Each patient's recovery varies based on procedure extent, individual healing factors, and adherence to post-operative instructions. During your consultation, we'll discuss personalised recovery expectations based on your specific situation.

Both a bad facelift and a bad toupee suffer from the same fundamental flaw: they're trying so hard to convince you they're natural that they end up screaming the opposite.

The tragedy is that both procedures, when done well, should be almost invisible. The best facelift is the one that has people saying, "My goodness, you look well-rested!" rather than "Who did your work?"

I often tell patients that the goal isn't to turn back time like some sort of surgical time machine, but rather to help them look like the best version of themselves – as if they've just returned from a particularly rejuvenating holiday, perhaps somewhere with excellent lighting and a skilled photographer.

The real artistry lies in restraint. Just as a toupee should move naturally with the wind and complement one's features rather than dominate them, a facelift should enhance rather than transform. We're not trying to create a new person; we're simply helping the existing person look as refreshed as they feel inside.

"Board-certified" only provides protection when the certification is in plastic surgery specifically. In South Africa, look for HPCSA registration as a plastic surgeon and APRSSA membership. Internationally, certifications like the American Board of Plastic Surgery or the UK's GMC specialist register for plastic surgery indicate comprehensive training.

"Cosmetic surgeon" isn't a protected title and doesn't indicate standardised training in plastic surgery procedures. Physicians from various backgrounds (dermatology, general practice, even gynaecology) can market themselves this way after weekend courses.

This distinction particularly matters for invasive procedures where complications can be serious. Don't hesitate to directly ask about a surgeon's specific board certifications and where they completed their plastic surgery residency.

Here are the facts...

Liposuction is a body contouring procedure for localised fat deposits, not a weight loss method or substitute for healthy lifestyle.

This persistent misconception leads to many disappointed patients. Liposuction is designed to refine body contours by addressing stubborn fat deposits resistant to lifestyle measures—not as a solution for obesity or general weight management.

Here's why the distinction matters:

• Liposuction typically removes only 2-5 litres of fat (approximately 2-5kg) for safety reasons
• The procedure doesn't address visceral fat (around organs) that poses health risks
• It doesn't prevent future weight gain in untreated or treated areas
• It doesn't improve metabolic health markers like blood pressure or cholesterol

The best candidates are within 10-15kg of their ideal weight with good skin elasticity and specific areas of disproportionate fat that haven't responded to targeted exercise.

When approached with appropriate expectations, liposuction can provide excellent contour refinement—but it complements rather than replaces a healthy lifestyle.

The journey to reclaiming your body shape after significant weight loss often has a final chapter that diet and exercise alone can't write. Body contouring procedures can help complete your transformation by addressing excess skin that remains after the weight is gone.

Body contouring procedures address weight loss concerns by removing excess skin and residual fat deposits while improving underlying muscle structure when needed. Depending on your specific concerns, options might include abdominoplasty (tummy tuck), circumferential body lift, thigh lift, arm lift, or breast reshaping.

Timing is important—these procedures are best performed after weight has stabilized for 6+ months. Candidates should be non-smokers with realistic expectations and good overall health.

Recovery typically requires 2-3 weeks away from work, with complete activity resumption by 6-8 weeks. Results continue refining for up to a year.

Thought it was time to properly introduce the people you'll meet when you visit our practice. Here's who does what from left to right:

Mariette runs the show as our Practice Manager. She's the one making sure appointments run on time and everything flows smoothly behind the scenes.

Cheryl keeps our admin sorted and is usually your first point of contact. She knows the practice inside and out and can answer pretty much any question you throw at her.

Me (Prof Sofianos) – I handle the surgical side of things. Outside of work, I volunteer with Operation Smile helping children with cleft conditions.

Karen (based at Lux Clinic) is our Registered Nurse who'll look after you during procedures and recovery. She's got the experience and the gentle touch that makes all the difference when you're feeling a bit nervous.

Nikkita (based at Lux Clinic) specialises in aesthetic treatments – the non-surgical options that can make a real difference without going under the knife. She's got a great eye for what works and what doesn't.

We've been working together for years now, and honestly, I couldn't ask for a better team. Each person brings something different to the table, but we all share the same goal: making sure you get the best possible care and results.

Thanks for trusting us with your treatment. It means everything to us.
