GSM - Genitourinary Symptoms of Menopause is a collective term for several bothersome niggles that start to occur during perimenopause and can remain at/after menopause.

GSM is actually one of the earliest signs of perimenopause because of how sensitive this area is to the slow decline in estrogen.

Which area are we talking about ~ the va**na, l***a, bladder and urethra.

Symptoms under the GSM umbrella include:

☂️ Dryness and decrease lubrication.
☂️ Itchy, irritable and burning of the v***a
☂️ Bladder irritability ~ constant need to wee or feeling like you haven’t emptied.
☂️ Bladder infections
☂️ Leaking urine
☂️ Pain with in*******se.

As estrogen declines these tissues thin out, become more inflamed and fragile. The ph of the va**na also changes which means the happy mircobiome shifts.

Things that can help :

• Topical va**nal estrogen (available OTC)
• Vaginal moisturizers with HA in it.
• Lubrication for in*******se.
• Topical Testosterone (prescribed by medical Dr)

April is C section awareness month 🤍

Many women are under the impression that having a c section means that are spared of certain pelvic niggles that only natural delivery can cause.

There is also a huge lack of support and info for ladies not only to be prepared for a c section birth but also to recover from one.

What does the pelvic floor have to do with intimacy and in*******se?

Well quite a lot actually.

The pelvic floor is a group of muscles at the base of the pelvis that are meant to relax during intimacy and arousal so that pe*******on can occur - not only comfortably but it also must be pleasurable.

During climax the pelvic floor rhythmically contracts to maximize the ultimate level of sensory pleasure.

Tension and weakness in these muscles could be impacting intimacy and derailing this process. This results in a negative cycle of pain, decreased arousal and loss of desire.

Pain and discomfort during intimacy is a big reason many ladies don’t find Seggs much fun at all. It’s more common than what is spoken about but that doesn’t make it normal or something anyone has to settle for.

Decreased Vag lubrication is a big one when it comes to things not feeling all that good down south. But the good news is there are easy things to do to help fix it.

First step understanding why…

⚡️ Lubrication be hormonally driven - one of the earliest signs of perimenopause is deceased natural lubrication.

⚡️ Lubrication also declines in the second half of the menstrual cycle when we are less fertile.

⚡️ On a hormonal contraceptive? This can also stagnate natural s*x drive and decrease arousal.

⚡️ Stress! If your body is constantly unregulated and in over drive it is not going to prioritize pleasure.

⚡️ Other drugs - certain meds can decrease libido and ultimately lubrication.

⚡️ Lack of sleep and exercise - again these help the body rest and restore. This allows more space for our s*x drive to thrive and want pleasure!

⚡️ Your relationship - are you communicating your needs and concerns with your partner?

⚡️ The pain cycle - expecting pain from an activity is not going to motivate anyone or anything to partake fully.

Massaging the oil into the tissue not only stimulates the tissue and improves blood flow but it can also help desensitize any up regulated nerves. It’s also a great way for a partner to be involved. For-play is a huge part of the journey.

Claire Bracher , professional nurse, midwife, lactation consultant and perinatal health coach . After a 10 year stint in academia she returned to clinical practise and was struck by how stressed and anxious new parents had become. She saw how negatively this was impacting their early parenting experience this awareness led her to train as an integral life coach through UCT and to offer coaching to pregnant and new parents. Claire works in group settings or with individuals and has had a profound impact on supporting new moms in finding more support in the new normal and chaos of early mommy life.

This AI thing looked like fun so here I am according to chatGPT 🤍

I am Kirsty Nel- Kirst to most. I am a pelvic/women’s health physio based in Joburg South Africa.

My focus is purely all things female however I do see the odd patient’s husband who feels like they have stepped into some secret club and have privy to the inner world of all things women.

Next month will be 7 years in my own practice and at the end of this month I renew my lease for another 3 years.

When I started on my own it was meant to just be a few hours a day kinda gig and I hardly looked at my instagram. However that quickly changed when I unexpectedly fell pregnant with my second child, went through COVID lockdown with a newborn + toddler and kept a baby practice a float. Somehow within that space of uncertainty my practice grew 🌱 and I grew.

I am now blessed to do the work I enjoy, still be a mom to my kids busy schedule, teach others interested in the same field, run a class to support moms in a space that I once was, give talks and lectures and so much more than the few hours a day that I had in mind initially.

My day to day practice focuses on abdominal and pelvic rehab post pregnancy, child birth and surgery.

I work with a lot of athletes and runners who battle with hip/groin pain.

I dry needle everything that can be needled! (With consent of course)

Fertility support with Accupunture is also a big part of my work. Along with chronic pelvic pain issues.

Oh and I love c section scar management - the more complicated the better for me 🤍

My practice is very pink and has lots of va**nal/boobs and vi*****rs models on full display (why the guys come back I am sure) It also always has something nice smelling in my treatment room!

Outside of my practice I run educational webinars, teach module 3 of the women’s health course, mom my two boys and run a weekly postpartum group .

Life is full and beautiful at the moment - I educate and empower women daily to take the plunge, uplift themselves and live their best lives!

Let’s be honest nobody likes going for one of these but Pap smears are a very important screening tool that women need to be aware of and use.

Cervical cancer is caused by naughty cellular changes in the cerical tissue. HPV is the culprit here and the truth is we are all exposed to HPV at some point in our lives. Sexual in*******se is an obvious transmitter but there are other ways too. Generally our immune system fights it off but certain strains are precancerous and early intervention is key to nipping it quickly.

Pap smears are done every 1-3years depending on what your Dr says and your risk profile. A GP can do a Pap smear so you don’t have to see a Gynae for this.

The va**nal canal is stretched open as the speculum is inserted and this can be very uncomfortable. A long tiny brush is inserted to collect a sample from the cervix. Kinda like how the throat COVID test was done but this is down under.

Here are some ways to navigate this with a bit more ease:

~ slow breathing down to regulate better. The faster you breathe the more sensitive you will become.

~ I recommend 4x4 breathing : Inhale 4sec, hold 4sec, exhale 4sec, hold 4 sec and repeat until you feel more relaxed.

~ if you are worried about lubricant take one with that you like and feel comfy to use.

~ relaxing your pelvic floor can also help. Think about letting go of your a**s - like relaxing for a fart 🙊 you won’t actually fart.

~ try and focus on keeping your regular breathing abdominal as this helps relax the pelvic floor.

~ let your Dr know to take it slowly.

~ remember the reason why you are doing it and the health benefits for that.

Keep your lady bits happy and healthy! If low risk this only happens every 2-3 years 🤍

Running is amazing and very popular way to blow off some mental steam. And there is no better time like postpartum to want to have options to navigate mental wellness.

But before you hit the road take a moment to think about a few things?

1. Slow and steady at first gets you further in the long term.

2. Starting with gentle movements, breathing and stretching helps the body reconnect in a safe space.

3. If there are any niggly symptoms those need to be seen to first.

4. Don’t underestimate walking and building distance, fitness and strength there first.

5. Recovery also includes the 9months of pregnancy prior to the birth trauma - give yourself time.
