Most documentation failures don’t happen because vets are careless.
They happen because vets are tired.

The profession asks clinicians to operate under extraordinary emotional and cognitive pressure — and still maintain perfect records. But when you’re juggling emergencies, overbooked consults, distressed clients, and chronic fatigue, even the best documentation habits erode.

And that’s exactly where medico‑legal risk grows.

In complaints across South Africa and Australia, one pattern appears again and again:

The vet remembers the conversation.
The notes do not.

Regulators rely heavily on contemporaneous records. They show consent, communication, reasoning, and professionalism. They protect you from shifting memories and hindsight bias.

But delayed or vague notes — “discussed options with owner” — offer no protection at all.

The fix isn’t perfection. It’s structure:
• Capture key discussions in real time
• Record refusals, risks, costs, and referral options
• Avoid vague phrasing
• Use templates
• Finish notes before fatigue blurs the details
