Seborrheic dermatitis, the underlying condition that leads to dandruff, can also be caused by Malassezia yeast. Although Malassezia colonies are found on every person's body, some individuals end up with too much of it. The condition can't be cured, but it can be managed with treatment - normally using an antifungal shampoo in combination with a topical steroid.

Know Your Diagnosis: Venous Stasis Dermatitis

Venous stasis dermatitis is normally seen in older patients with an underlying heart disorder, although not exclusively so. Patients will normally complain of swelling in their legs, which results in a red, brown or even black discolouration of the skin. The skin can also become hard and thick with very prominent veins and intensely itchy, scaly plaques.

If your eyelids get itchy when you use a particular brand of makeup, you may suffer from allergic contact dermatitis.
Women tend to get allergic contact dermatitis on their eyelids if they are sensitive to a certain ingredient in the makeup they use. If you suspect you have this kind of dermatitis, this is one of the indications for allergy testing - a patch test in particular - to confirm the diagnosis.

If your earlobes itch when you wear a certain pair of earrings it could be allergic contact dermatitis.
Allergic contact dermatitis occurs when the skin comes into direct contact with an allergen. This activates the body's immune system, which triggers inflammation. Symptoms usually include intense itching and a raised, red rash or even blistering. This is limited to areas of the skin that comes into direct contact with an allergen. A common example is a nickel allergy, which causes a rash on the earlobes when it comes into contact with earrings that contain this metal, on the fingers when it comes in contact with rings, or the lower abdomen with it comes in contact with a button or a belt buckle.

If your skin goes red or dry when you use a cleaning product, you may suffer from Irritant contact dermatitis.
Irritant contact dermatitis occurs when the skin comes into direct contact with a substance that physically, mechanically or chemically irritates the skin and causes disruption of the skin barrier. The most common causes of this type of eczema are things we use on a daily basis like soap, cleansers, or rubbing alcohol. A mild irritant will cause redness, dryness, cracking, burning, stinging or itching. Strong irritants, on the other hand, cause swelling, oozing, tenderness or even blisters. The areas that are most commonly affected are the hands and face, especially the eyelids.

Interesting fact - cradle cap is a form of eczema.

Seborrheic eczema is a common type of eczema. There are two distinct variants. The one that we see in newborn babies that presents with redness and greasy scale is also known as cradle cap. It is normally found on the scalp, but it can also affect the face, ears, neck, diaper area and skin folds. In adults, it is found on the scalp, as well as the eyebrows, around the nose, in the crease between the nose and the lip, or the central chest, sometimes presenting in the form of a greasy scale. Dandruff is also mild form of seborrheic dermatitis.

Know Your Diagnosis: Atopic Eczema

Atopic eczema is the most common type of dermatitis. The most prominent symptom is severe itching in the form of red, scaly plaques or small, red bumps. In infants, it's most commonly found on the cheeks or the front of the arms or legs. In older children and adults, we see it in elbow creases or at the back of the knees. In older adults it can also present on the hands and the eyelids.

Your safety is of utmost importance to us and therefore we have placed the necessary preventative measures in place.

Please remember to wear a mask when visiting our practice. Where possible, patients are requested to attend consultations on their own (exception: parents of minors).

If you have ever tried using antihistamines to cure the itch of eczema we bet it wasn’t too effective, and there’s a good reason for that.

Histamine is not the mediator that causes the itch in eczema. A chemical itch mediator is a substance in the body that acts as a messenger. When introduced into the skin, the chemical mediator acts on nerves endings in the upper layers of the skin, or indirectly through cells that play a role in causing the itch. In the case of eczema, this mediator is Interleukin-31.

As such, antihistamines do very little to reduce the itching of eczema. We would normally only prescribe sedating antihistamines for patients with eczema to help them sleep in the evening, or if they experience concurrent sinusitis or hives.

If you have been told that you need to go for allergy testing after you’ve received an eczema diagnosis we have some good news – this extra step could be unnecessary.
Supplementary testing is not necessary in the vast majority of cases, as most often eczema is a clinical diagnosis. In a small sub-set of patients with very clear, well-defined criteria one might consider doing blood tests, skin prick tests, patch tests, or a punch biopsy.

Have you ever been told that cutting out gluten is a cure-all solution that will solve your eczema?
We’re here to tell you that this is not the case. The European Academy of Dermatology and Venereology 2018 guidelines clearly states that there is no place for routine elimination diets in the management of eczema, since it is caused by skin abnormalities and defects which are completely unrelated to diet in the vast majority of cases.
The best long-term solution for eczema is to work with a dermatologist to develop a workable treatment plan.

The reasons behind your eczema may not be what you think it is…

One of the most common misconceptions about this skin condition is that it is caused by allergies. This is false in the vast majority of cases. The medical term for eczema is 'dermatitis', which means inflammation in the skin. There are different types - each caused by different skin abnormalities and defects. For instance, the most common kind of eczema, atopic dermatitis, is caused by a hereditary barrier defect in the skin.
The best way to get to grips with eczema is to visit a dermatologist so you can get to work with an optimal treatment plan.

Dr Jacques van Wyk Dermatologist: National lockdown statement.

In the mist of the Covid-19 Pandemic, this practice will, in accordance with national guidelines, implement the following measures for the duration of the 21-day lockdown period:
1.) In order to take pressure off local emergency centers, we will still be seeing urgent cases.

2.) If you feel that you have an urgent skin problem, please contact the practice via email, info@jacquesvanwyk.co.za or phone 021 007 3771 to make an appointment.

3.) Selected non-urgent cases can be managed via video consultation. These consultations will be available to anyone in residing in South Africa but will be limited to certain conditions. Unfortunately, these services will not be available for conditions relating to routine mole checks, skin cancer screening, keratosis, sun damaged skin or evaluation of suspicious skin lesions. Please send an e mail to info@jacquesvanwyk.co.za to enquire and/or book a video consultation.

4.) We will return to full normal service on 17 April 2020 (or as soon as deemed safe thereafter). Please feel free to book a consultation in advance on 021 007 3771, the telephone line will be fully operational during the lockdown.
