Born and bred in Paarl, Ghéte Pegram has always had sport at the center of her world. Whether she's courtside at a tennis match or in the treatment room working with a patient, her passion for movement runs through everything she does.

Her background in sport gives her a real understanding of what it means to be sidelined and what it takes to get back to doing what you love. She has a particular interest in orthopedic conditions and brings that same drive to staying at the forefront of evidence-based practice.

Managing a frozen shoulder?

One of the most common things we hear from patients is that they were told to just wait it out.
The problem isn't the waiting. It's what happens during it. Months of disrupted sleep. Gradually avoiding certain movements without realising it. The neck, upper back, and opposite shoulder starting to compensate, creating a whole new set of problems on top of the original one.

Physiotherapy works with the natural process, not against it. It manages pain at each stage, keeps movement from deteriorating further, and helps the body move through recovery more efficiently. Research indicates that early physiotherapy and consistent movement can significantly reduce the overall duration of frozen shoulder by up to 30%.

Frozen shoulder (adhesive capsulitis) is more than just a stiff and painful joint. It occurs when the connective tissue surrounding the shoulder joint becomes inflamed, thickened, and tight, leading to a progressive loss of mobility.

It's a condition that changes over time, and the way it's managed needs to change with it. The freezing stage brings increasing pain alongside stiffness, and can last 2 to 9 months. The frozen stage is where movement is most restricted, even though pain may start to settle. This stage typically lasts 4 to 12 months. The thawing stage is where the shoulder gradually loosens. This process can take 5 months to 2 years.

Risk factors include diabetes, thyroid conditions, and periods of shoulder immobility after surgery or injury. Many people don't connect these dots until the stiffness has already set in.

Physiotherapists can help with management in all stages of frozen shoulder, but are specifically most helpful in the frozen and thawing stage to improve shoulder mobility.
