It’s important to know the difference between gagging and choking.
This is because intervening inappropriately when a child is gagging can actually increase the risk of choking.
And while gagging is NOT choking and gagging does not lead to choking, if a baby is startled by a caregiver while gagging or if a caregiver intervenes in a way that interferes with the body’s natural reflex of gagging, it can make the situation worse.
Gagging is a protective reflex that closes the airway to bring things back up and out of the esophagus and mouth. And from our perspective as infant swallowing specialists, gagging can actually be helpful while babies learn to move food around in the mouth.

Back to gagging. Gagging often sounds like the beginning of a vomit - retching, but may also be silent - and the tongue is usually cupped and mouth open in a long “0” shape. Gagging is easily triggered in babies, and remarkably protective against choking.
True choking, however, is when the airway is obstructed and baby can’t breathe or cough, requiring someone to intervene to get the item out. Signs of choking can include:
🚨Inability to cry OR cough
🚨Difficulty breathing
🚨Skin tugging into the chest
🚨Look of terror
🚨High-pitched sounds (wheezing, whistling)
🚨Skin color changes (blue to purple to ashen-like)
Choking is a medical emergency. If you suspect your child is choking, administer age-appropriate choking first aid and call emergency services on speaker phone so your hands remain free. Avoid slapping or patting the child’s back while they are seated (proper back slaps should have the child down on an angle, see our free guide for pictures or our course for videos demonstrating it) and never put your finger in baby’s mouth, this often results in pushing the object/food further back.
