CRITICAL MOMENTS

Yesterday I had a chilling reminder of how precious this thing called life is, and I want to share my thoughts to you as a patient, but also to every person out there that practices outside their scope and realm of expertise. If you are offended by this post as a practitioner then I have achieved my goal today and I invite you to contact me and discuss the details in person.

Yesterday a perfectly healthy, young woman walked into my consultation room complaining of calf pain....

I want to stop there and highlight 2 very important things to patients and "practitioners" out there:

1. If you have PAIN associated with a possible injury or pathology you need to see an appropriate professional healthcare practitioner that can evaluate and diagnose you. That includes a medical doctor or a qualified physiotherapist. When it comes to musculoskeletal injuries where pain is still present you need to consult someone trained and educated to deal with injury, pain and pathology.

The following practitioners are not trained to evaluate with the aim of diagnosis, and are not scoped to work in the presence of pain; sport scientists, biokineticists and all other unregulated "sports or massage therapists".

2. To practitioners out there - stop, listen and evaluate your patients properly. If you neglect your subjective examination you are playing with fire, and you will end up missing vital information.

Back to our patient - subtle hints revealed that this healthy individual sitting in front of me was a high risk for a deep venous thrombosis and even had very subtle signs of a pulmonary embolism.

Needless to say an immediate referral to the emergency room confirmed the presence of a DVT and some clots already in the lungs.

These are critical moments that you can end up missing if you as a practitioner get lazy and complacent.

So my advice today is two-fold:

1. To practitioners out there - stop this money making mentality where you only spend 15 minutes with a patient.

2. Stop playing with fire outside your scope of practice.

Somewhere between Inchanga and Botha's hill on a Comrades training run...

This weekend will see most runners take on their last long training run for Comrades.

To get the most out of your run, don't forget the following:

- Don't forget the principles of training specificity. Do your run on a hilly course, simulating the Comrades course.

- Test all your gear, nutrition products and fueling strategies this weekend. You want to make sure that everything works for you this weekend under long run conditions.

- No need to go and run 80kms this weekend. We feel that anything between 50 and 60 kms should be enough.

- Make sure you have a proper recovery plan in place afterwards.

- Have fun!

When it comes to your health and well-being there's so much more involved than what is visible on the surface. You are not only a physical being with skin, muscle, tendon and bone.
