We had two recent endoscopic courses in association with Wits, Zuid Afrikaans hospital and Spineafrica academy. The the first course was a basic course and the second an anvanced transforaminal course. Dr Satishandra Gore, conducted numerous lectures on behalf of Mission Spine, a non profit organisation, and covered all basic subjects, as well as his transforaminal new system, SMRUTII, for spinal stenosis. Life surgeries conducted at Zuid Afrikaans hospital included lumbar as well as thoracic cases and by using O arm navigation, principles were more easily explained and shown with precision, and helped the participants with orrientation. We thank all the different endoscopic companies, which included Storz, Elliquince and Maxmore for ther participation, which helped to introduce surgeons to different tools and systems. We also thank the local and international participants for the effort in attending, and are looking forward to the next course later in the year.

The need for surgeon training is much needed . Surgeons can provide a higher level of care incorporating modern technology into their practice. Navigation with newer minimal invasive techniques like camera assisted microsurgery and endoscopy, will lead to only pathology being addressed and less morbidity. Other techniques and procedures that need to be adressed are Total Disc Replacement, Dynamic forms of instrumentation , and all the different approaches to the Spine.
All these are totally lacking in Tertiary institutions. Surgeons travel for weekend courses to obscure institutions ,and come back as “experts” with non optimal results.

Endoscopic fusion done through a 8 mm incision. All instrumentation are navigated. The future of spinal surgery will be endoscopic , computer guided and robotic. This will lead to most spine surgeries being done in day hospitals with same day discharge, and early return to normal function.

In this strange times of COVID, we are forced to change in many ways . Internet consultations will become more popular, but is not ideal. Wer aer still consulting at Club surgical centre, where the the flow of people are minimal. We are sceduling appointments on the hour to only have one patient at a time at the rooms.
