Deist Physiotherapists strive to provide the highest quality physiotherapy service to all categories of patients, referring practitioners and the community. They are a team of of passionate and experienced physiotherapists with a special interest in pelvic health and lymph-node treatments. g conditions, back and neck problems and sports injuries. Isabel has been in private practice for the past 19 years and a private practice owner for 15 years there of. She has a special interest in pelvic pain and pelvic floor rehabilitation and has been part of the My Sexual Health team for the past 7 years. In 2008, Deist Physiotherapists also became part of the My Sexual Health team, treating pelvic pain and incontinence. This has since grown into a pelvic rehabilitation centre treating anorectal (anal/rectal) problems, genitourinary system or urogenital system (reproductive/urinary) problems, and pre and post-natal (pregnancy) patients. Talks at the hospital on pelvic issues during and after pregnancy have also been held. At present a new scope has been added to our services, namely lymphatic drainage. The treatment areas Deist Physiotherapists offers, include:

- Back and neck conditions
- Post operative orthopaedic rehabilitation
- Lung conditions
- Sinusitis
- TMJ (jaw) pain
- Lymphatic drainage
- Pre and postnatal (pregnancy) biomechanical pain
- Pelvic floor rehabilitation – incontinence (unable to retain urine),
(male and female)
- Vaginismus (va**nal spasms), vulvodynia/ vestibulodynia (pain in the
opening of the va**na),
- Chronic pain
- Dry needling

The practice is aiming to establish a pelvic rehabilitation centre well known in the area, promoting physiotherapy services that the public may not always associate physiotherapy with. This practices is a place where gynaecologists, urologists and colo-rectal surgeons can refer their patients with complications post-surgery or that can not necessarily be treated with surgery. Having an in-house physiotherapist with international training in lymphatic drainage, Deist Physiotherapists are also aiming to provide a service in Pretoria East for patients, that currently need to travel to other hospitals or practices further away from the area. We welcome invites to talks, on educating the public on all services rendered by physiotherapists and on different conditions and to write for media publications on our treatment offerings. One of the key goals in pelvic floor training is improving sphincter control.

This is closely aligned with the concept of motor control, which is the ability to activate muscles at the right time, in the right sequence, and to the appropriate level for the task.

In the pelvic floor, this means the sphincter system must respond dynamically. It needs to increase tone when support is required and reduce tone when relaxation or movement demands it.

While Kegels can be a useful tool to build awareness and strength, they often fall short because they do not address how the pelvic floor functions as part of the whole body.

Pelvic floor activity is influenced by factors such as posture. For example, an upright posture can increase pelvic floor tone, whereas a slouched posture can reduce it.

This highlights the importance of the connection between the spine and the pelvic floor.

By working with posture and movement, we can influence pelvic floor activity without relying solely on conscious contractions, which have limitations.

The pelvic floor is not an isolated structure. It is part of a pressure regulation system that must coordinate with the diaphragm, abdominal muscles, and spine.

Effective training should reflect this complexity and focus on adaptability rather than just strength.

As we wrap up the latest cohort of the Fit Floor programme for Pilates teachers, it has been a great opportunity to explore how we can improve both pelvic floor training and the way we communicate dynamic pelvic floor concepts to clients.

I have thoroughly enjoyed this group and look forward to running the programme again later this year.

These days treatment for HIV (antiretroviral therapy) is highly effective.

Antiretroviral therapy is not a cure, but people diagnosed with HIV who start treatment as soon as possible can expect to live long and healthy lives. The treatment is usually simple to take, has few side effects and is very robust in controlling/suppressing HIV, but it does mean taking it for life.

The following tests are commonly performed in the HIV outpatient setting. The CD4 blood test, refers to your CD4 cell (or T cell) count, and is a rough measure of your immune system.

The CD4 helper T-lymphocyte is the predominant immune cell that the HIV virus infects and destroys. Over time the body can’t effectively replace these lost CD4 cells and the size and function of this cell population declines and as it does so, the ability of your immune system to fight infection will also deteriorate. The lower the CD4 count the less able your immune system is to fight infection and the more serious and extensive the range of infections you become susceptible to.

The HIV viral load is a measure of how much replicating HIV is found in the circulating blood.

In a HIV positive person this usually varies from a few hundred to several million copies/ml. In patients on HIV treatment, the viral load is an accurate measure of how effective treatment is and the treatment aim is for the viral load to reach an "undetectable" level. Treatment not only prevents you developing AIDS but significantly reduces the transmission risk of passing the virus on to your partner(s).
