Not all “swollen lymph nodes” mean the same thing
Understanding the difference matters — especially for women.

🔴 INFLAMED LYMPH NODES
• Usually caused by infection or inflammation
• Feel tender or painful
• Often sudden swelling
• Common with flu, colds, throat or dental infections
• The node itself is reacting to illness

👉 This is your immune system doing its job 💪

🟡 BLOCKED / CONGESTED LYMPH NODES
• Caused by poor lymphatic flow
• Usually not painful, but uncomfortable
• Feel firm, full, or heavy
• Often linked to lymphatic dysfunction, lymphedema, lipoedema, post-surgery, chronic swelling
• The lymph fluid isn’t draining properly

👉 This is a drainage issue, not an infection.

🩺 A note on dense breast tissue
Women with dense breast tissue may experience swollen or congested lymph nodes, especially in the armpit and breast area.
This can feel like:
• Tenderness & Fullness
• The armPIT does not dent in
• Lumpiness or heaviness

➡️ these nodes need to be assessed to rule out inflammation or other causes and to support healthy lymphatic flow.

You can have anorexia and STILL suffer from lipoedema...

Lipedema / lipoedema fat is fundamentally different from normal body fat.
The differences are structural, hormonal, inflammatory, and functional.
This is exactly why a lipedema fat cell does NOT respond like ordinary fat cells to diet and exercise!

Below is a clear, clinical-style comparison:

1. Origin & Purpose

Normal fat cells (adipocytes):

* Store energy (calories) for later use
* Provide insulation and protection
* Increase or shrink depending on calorie balance
* Distributed evenly over the body

Lipedema fat cells:

Pathological fat tissue (a disease process)
These cells grows disproportionately, mainly in:

* Hips
* Thighs
* Buttocks
* Legs
* Sometimes arms
*Spares the feet and hands and has the classic ankle cuff and wrist cuff --> key diagnostic clue
*Growth is not driven by calorie excess**

2. Cell Structure & Size

Normal fat cells:

* Relatively uniform in size
* Expand (hypertrophy) and shrink predictably
* Normal cell membrane elasticity

Lipedema fat cells

* Enlarged, fibrotic, and irregular
* Often clustered in nodules
* Cell membranes are stiff and dysfunctional
* Reduced ability to shrink once enlarged

👉 This is why lipedema fat feels lumpy, nodular, and painful to touch ( especially when going for a normal swedish an sports massage)

3. Hormonal Sensitivity

Normal fat

* Responds to insulin, cortisol, and adrenaline
* Burns fat during calorie deficit and exercise

Lipedema fat

* **Highly sensitive to estrogen*
* Often triggered or worsened by:
* Puberty
* Pregnancy
* Postpartum
* Perimenopause / menopause
* Poor response to lipolytic hormones when in ketosis

👉 This explains why many women notice rapid progression during hormonal shifts, even without weight gain.

4. Blood Vessels & Capillary Fragility

Normal fat

* Normal microcirculation
* No bruising

Lipedema fat

* Extremely Fragile capillaries
* Increased permeability (swelling)
* Easy bruising
* Localized swelling that worsens during the day

👉 Blood and fluid leak into surrounding tissue, contributing to pain and inflammation.

5. Lymphatic Involvement

Normal fat

* Minimal lymphatic stress

Lipedema fat

* Compresses the lymphatic vessels
* Slows lymph flow
* Can progress to *lipo-lymphedema* if untreated

👉 This causes heaviness, pressure, and later true swelling.

6. Inflammation & Pain

Normal fat

* Low-grade inflammation only in obesity
* Usually painless

Lipedema fat

* Chronically inflamed ( with blood tests, inflammatory markers are always elevated)
* Nerve irritation inside fibrotic tissue
* Pain, tenderness, pressure sensitivity
* Pain not relieved by elevation

7. Response to Diet & Exercise

Normal fat

* Reduces with dieting
* Exercise leads to fat loss and muscle tone

Lipedema fat

* **Extremely Resistant to fat loss**
* Weight loss affects:

* Face
* Upper body
* Waist
* Affected areas remain disproportionately large

👉 This often leads to misdiagnosis as “noncompliance” or obesity, which is incorrect and harmful.

8. Texture & Clinical Feel

Normal fat

* Soft
* Even
* Compressible

Lipedema fat

* Grainy, nodular, fibrotic
* Feels like:
* Rice
* Peas
* Walnuts (depending on stage)
* Very Tender or painful to squeeze

9. Disease Progression

Normal fat

* Stable if lifestyle remains consistent

Lipedema fat

* Progressive
* Worsens without management
* Can lead to:

* Joint strain
* Mobility issues
* Secondary lymphatic failure

💦 The difference between Lipoedema and other lymphatic conditions 💦 and why proper assessment of leg swelling really matters:

In my practice, I meet many women who share a similar concern:
their legs feel heavy, often have some degree of swelling, fatty lumps, tender tissues, bruise easily, and continue to enlarge despite healthy eating and regular exercise.

This understandably leads many to suspect lipedema — but in reality, not all leg swelling or fat distribution is the same.

Several medical conditions can appear very similar on the surface, yet they require very different management approaches. Understanding the distinction can prevent years of frustration and delayed care.

🌸 Lymphedema 🌸
= fluid build-up due to lymphatic dysfunction.

Lymphedema occurs when lymph vessels are damaged (usually due to surgery or injury) , lymph node removal (such as in the case of cancer) causing fluid to accumulate in the tissues.

Signs and symptoms: heaviness, swelling, pitting oedema and a feeling of very tight or thickened skin.

🩺 *Clinical clue:* in the case of lower limb lymphoedema, swelling may be uneven, and the skin at the base of the toes is often difficult to lift (Stemmer sign).
Without proper management, tissue hardening and recurrent infections may develop.

🌸Lipedema 🌸
= This is a painful fat disorder, with accumulation of lymph fluid

Lipedema is a chronic condition affecting fat distribution, primarily in women. It typically presents symmetrically in the hips, thighs, knees, calves, and sometimes the arms.

🩺 *Key signs:* tenderness to touch, easy bruising, and feet & hands that remain unaffected.
Many women notice that weight loss reduces the upper body, while the legs and arms remain unchanged. Over time, untreated lipedema may also affect lymphatic flow, leading to lipo-lymphedema.

🌸Chronic Venous Insufficiency🌸
= poor blood return from the feet (up against gravity) in the legs.

When leg veins don’t function efficiently, blood pools (build-up) in the lower limbs, causing the water content of the blood to leak through into the surrounding tissues, resulting in oedema (swelling)

🩺 *What to look for:* swelling that worsens as the day progresses, aching, visible varicose veins, and skin darkening near the ankles.
Advanced cases may result in chronic wounds if left untreated.

🌸Dercum’s Disease🌸
= localized painful fat deposits

This rare condition causes multiple painful fatty nodules (lipomas).

🩺 *Difference:* pain is usually confined to specific lumps rather than being evenly distributed, as seen in lipedema.
Because it’s uncommon, it is frequently misdiagnosed.

🌸 Weight-related swelling 🌸

= obesity related swelling

Excess body weight place pressure on veins and lymphatic vessels, leading to leg swelling.

🩺 *Distinguishing factor:* swelling often improves with weight reduction and increased movement — unlike lipedema, where abnormal fat tissue persists.

🌸Thyroid-related swelling🌸 (Myxedema)

An underactive thyroid can cause generalized puffiness rather than localized limb swelling.

🩺 *Often accompanied by:* fatigue, cold sensitivity, dry skin, and unexplained weight gain.
Appropriate medical treatment usually improves these symptoms.

🌸Deep Vein Thrombosis (DVT)🌸 – a medical emergency

DVT involves a blood clot forming in a deep vein, usually in one leg.

🚨 *Red flags:* sudden one-sided swelling, warmth, tightness, and pain.
Immediate medical assessment is essential.

Symmetrical, painful legs with easy bruising → consider lipedema
Firm, uneven swelling → suggestive of lymphedema

Lipoedema fat is different.
It behaves differently, feels different, and affects the body differently.

Women with lipoedema often experience:

This day was really special 💚
I had the chance to teach a family member how to do lymph drainage on her sister, and how to apply multi-layer bandaging at home.

Sometimes, for some reason, coming to the practice every day just isn’t possible — life happens. Between work, family, and finances, it’s not always easy to fit in daily sessions, and that’s okay. What matters in this case is finding ways to keep the reduction of oedema going, even if it is at the patient's own home.

That’s why home-care and education is so important. Teaching loved ones how to help makes such a difference, especially during the acute phase of treatment. It’s about working together, being consistent, and empowering patients and families to take charge of their care.

As a nurse by heart, I always try to meet people where they are, go the extra mile in order to make the best of what we have — because every bit of effort counts, and it all adds up to healing. 🌿💫
