"Lenacapavir (LEN) offers a historic opportunity to reshape HIV prevention by substantially reducing the number of new infections and accelerating our progress towards ending AIDS. But this promise will only be realised if its introduction is managed with transparency, sustainability, and accountability. We must ensure that LEN’s rollout does not repeat past mistakes, does not undermine core prevention tools, and does not leave countries exposed to unsustainable costs. With responsible stewardship and significant community involvement, LEN can be a turning point in epidemic control; but without it, it risks becoming another short-lived experiment. During initial implementation, monitoring who initiates LEN and their associated risk levels will be essential to refine cost-effectiveness estimates and guide future expansion strategies," Dr Thembisile Xulu, SANAC CEO at the National Roundtable on Lenacapavir Access and Sustainability in South Africa.
