A doctor will issue a DHA-1663 form (Notice of Death) at the hospital. It is important to obtain this form.
Once you have the DHA-1663 form, contact a funeral home to make arrangements for the collection of the deceased.
The DHA-1663 form serves as an official document to facilitate the necessary procedures and allows the funeral home to assist you in carrying out the appropriate arrangements. It is crucial to have this form before contacting the funeral home.
