Today is World Obesity Day, a reminder to rethink how we talk about weight.

Weight gain is often framed as simply “calories in vs calories out,” but in reality it is influenced by many factors which include hormones, sleep, stress, genetics, medications, underlying health conditions and how metabolism adapts over time.

For some people, these systems can become dysregulated, making weight loss and weight maintenance far more challenging than many realise.

Type 2 diabetes doesn’t start with high blood sugar. It often begins years earlier with insulin resistance — a metabolic condition where your cells stop responding properly to insulin.

During this stage, blood glucose can still appear “normal,” while the body produces increasing amounts of insulin to compensate. This silent phase can last 10–15 years before diabetes is diagnosed.

And it’s not just about sugar.

Insulin resistance contributes to:
• Prediabetes
• Type 2 diabetes
• Metabolic syndrome
• Fatty liver disease
• Hypertension
• High cholesterol

Approximately 80% of glucose disposal happens in skeletal muscle — which is why resistance training and regular physical activity are so important in improving insulin sensitivity.

In South Africa, where cardiometabolic disease is rising, recognising this early stage is critical.
