Premature Cardiovascular Disease — What You Need to Know

Cardiovascular disease remains one of the leading causes of premature death in South Africa. Here in KZN, we see this reality in our consulting rooms every week.

Coronary heart disease is no longer only a disease of the elderly. In Tongaat, Durban, and surrounding communities, we are seeing younger patients present with heart disease; often before they ever considered themselves at risk.

Research from R.K. Khan Hospital in Durban found that among patients aged 45 and younger admitted with acute heart attacks, smoking and abnormal cholesterol were the most common risk factors. More than half also had a direct family history of coronary heart disease.

This is especially important in South Asian communities, where global data shows that 50% of heart attacks occur before age 50, and 25% before age 40.

Key risk factors we screen for include:

🔹 High blood pressure
🔹 Abnormal cholesterol
🔹 Type 2 diabetes
🔹 Smoking
🔹 Family history of heart disease
🔹 Abdominal obesity

What’s concerning is how often these risk factors go undetected. Many patients experience their first cardiac event without ever having been diagnosed, not because the warning signs weren’t there, but because they had not been screened.

After 14 years. 22,000 survey responses. 56 organisations. One landmark paper in The Lancet.

Yesterday, PCOS officially became PMOS.

Polycystic O***y Syndrome → Polyendocrine Metabolic Ovarian Syndrome.

One letter. Seismic implications.

Here’s why this matters more than people realise:

The old name was never accurate. Women were told they had “cysts on their ovaries” — but many never did. That single misconception cascaded into decades of delayed diagnoses, fragmented care, and a condition affecting 170 million women being treated as a reproductive footnote rather than the complex hormonal and metabolic disorder it actually is.

PMOS doesn’t just rename the condition. It reframes it:

→ It’s hormonal (polyendocrine)
→ It’s metabolic (cardiovascular risk, diabetes, weight)
→ It’s systemic (skin, mental health, fertility, long-term organ health)

Language in medicine isn’t semantics. It determines what gets researched, what gets funded, and critically — what gets believed.

This is a landmark day for women’s health. And it’s only the beginning.

🩺 Health Update: Hantavirus Case in South Africa

South African health authorities are monitoring a rare imported case of Hantavirus Pulmonary Syndrome (HPS) linked to international cruise travel. One patient is being treated in Johannesburg while contact tracing continues.

The case is believed to involve the Andes strain, the only hantavirus strain known to rarely spread between people through close contact. However, the WHO and NICD say the overall public risk remains low.

⚠️ What is hantavirus?
Hantavirus is a rare but serious viral infection usually linked to exposure to infected rodent urine, droppings, saliva, or contaminated dust in enclosed spaces. Most cases worldwide result from environmental exposure rather than human transmission.

⚠️ Symptoms to watch for:
Early symptoms may include:
• Fever
• Fatigue
• Severe muscle aches
• Headache
• Nausea, vomiting, diarrhoea, or abdominal pain

More serious symptoms can develop later, including:
• Cough
• Chest tightness
• Shortness of breath
• Difficulty breathing

The illness can worsen quickly and may require urgent medical care.

🚨 Seek medical attention if you develop flu-like symptoms and:
• Recently travelled to South America or affected cruise routes
• Had close contact with a confirmed case
• Were exposed to rodent-infested areas
• Develop breathing difficulties after a flu-like illness

✅ Prevention tips:
• Avoid contact with rodents and rodent droppings
• Ventilate closed spaces before cleaning
• Do not sweep dry rodent droppings; disinfect first
• Practise good hygiene while travelling or camping

The NICD says hantavirus infections remain extremely rare in South Africa, with no evidence of widespread local transmission.

Plain water is helpful, but during gastro it’s often not enough on its own.

When you’re vomiting or have diarrhoea, your body loses both fluids and important salts (electrolytes). Without those salts, the water you drink isn’t absorbed as effectively.

A mix like this helps your gut take in fluid more efficiently and keeps your system balanced.

Sip slowly, with small amounts at a time. Consistency matters more than volume.
