What are the layers of the skin?
1. Epidermis
2. Dermis
3. Hypodermis

1. Epidermis (Visible Part of the Skin):

Functions include:
- Protection
- Responsible for oneโ€™s skin tone
- A protection against bacteria and allergens.

Important cells and why they are important:
- Keratinocytes: Responsible for maintaining a waterproofing effect on the skin
- Melanocytes: Responsible for the Pigmentation of the skin
- Langerhans Cells: Protection of bodyโ€™s Immune system.

2. Dermis (Beneath the Epidermis):

- Made up of a thick upper layer (Papillary Dermis Layer): Which is important For regulation of heat loss and much more
- Made up of a thick lower layer (Reticular Dermis): Which is important in giving the skin its elasticity and strength.
- Responsible for Sweat Glands, Hair Follicles
- Contains tough connective tissue called Collagen fibers which provide durable strength to the skin.

3. Hypodermis

The Hypodermis contains fat and connective tissue which acts as an Insulator and padding.

Functions of the Skin

The skin is often referred to as the largest organ of the body. When the skin is damaged, it is important to cover the damaged area as quickly as possible, to prevent infection from occurring and allowing normal skin function to return.

The skin plays a vital role in wound healing using the following functionsโ€ฆ

1. Protection - the skin serves as the main protective barrier, preventing damage to internal tissues, as well as preventing harmful substances from entering the body like bacteria which may lead to wound infections.

2. Sensation - nerve endings present in the skin allow the body to detect pain, and changes in temperature, touch, and pressure. Wound pain is experienced when the nerve endings are exposed in open wounds and loss of sensation may lead to pressure injuries if pressure relief is not being followed correctly.

3. Thermoregulation - allows the body to respond to changes in temperature by constricting or dilating the blood vessels within, produces sweat which stays on the skin allowing the body to cool down, or raising the hair and trapping warm air next to the skin. Thermoregulation becomes challenging with burn wounds.

4. Excretory function - skin excretes waste products in sweat which contains water, urea, and albumin. A loss of albumin (protein) will delay wound healing.

5. Absorption - when UV light is present (mostly from the sun), the skin produces vitamin D which is required in the final stages (Proliferation) of wound healing to promote wound closure. Vitamin D also assists with the absorption of calcium to promote bone growth.

Factors Influencing Wound Healing

Factors that affect wound healing which interfere with one or more of the healing phases (Inflammation, Proliferation, Maturation), can lead to improper or impaired tissue repair.

1. Local Factors โ€“ Wound healing can be delayed by local factors that directly impact the wound:
โ— Poor Blood Supply
โ— Local Infection
โ— Desiccation
โ— Pressure, Shear, Friction
โ— Skin Maceration
โ— Topical Applications
โ— Foreign bodies - Necrotic tissue, Slough, Eschar, Cotton wool fibres, Sutures
โ— Wound Location, Size and Depth

2. Systemic Factors โ€“ Refers to the overall state of health of the patient, including underlying conditions that can affect wound healing:
โ— Nutrition

โ— Age
โ— Underlying Systemic Condition

3. Other Factors:

The TIME framework was developed as a practical guide for use when managing patients with wounds to assist with identifying barriers to wound healing and implementing a plan of care to remove the barriers and promote wound healing.

T โ€“ Tissue
An accurate description of the tissue is a very important part of the wound assessment. Non-viable tissue will delay wound healing, increase risk of infection, prolong the inflammatory response, and prevent the wound from healing.

I โ€“ Inflammation or Infection
All wounds contain micro-organisms or bacteria. There are 3 levels of Infectionโ€ฆ
โ— Colonized โ€“ Presence of nonreplicating bacteria with no clinical signs of infection
โ— Critically Colonized โ€“ Subtle signs of infection such as increased exudate, slough, or odour
โ— Superficial Infection โ€“ Non-healing, Exudate, Red/Bleeding, Debris, Smell (NERDS)
โ— Deep Infection โ€“ Level of microbial burden has caused injury to the surrounding skin or deep tissue. Size is bigger, Temperature Increase, Os (Bone) exposed, New areas of breakdown, Exudate, Erythema, Edema, Smell (STONEEES)

M โ€“ Moisture
Moist wound environments enhance wound healing and promote new tissue growth.
Excess moisture impairs the healing process and causes a breakdown of the wound bed and surrounding skin, leaving the wound at risk of bacterial infection.
Low moisture levels may lead to necrosis and eschar formation, preventing the wound from healing.

E โ€“ Edges
The final stage of wound healing is epithelialization โ€“ the active division, migration, and maturation of epidermal cells across the open wound.

Phases of Wound Healing

Wound Healing consists of 3 phases:
โ— Inflammatory Phase
โ— Proliferation Phase
โ— Maturation and Remodeling Phase

1. Inflammatory Phase:
This phase is a response of the body, to be protective and defensive.
โ— Haemostasis (Stop the leak) โ€“ Starts immediately after injury and lasts about 5 to 10 minutes. Platelets and fibrin stick together to form a clot to stop any bleeding.
โ— Inflammation (Calling the clean-up crew) โ€“ symptoms include erythema (redness), warmth, oedema (swelling), and pain. Lasts 4 to 6 days.

2. Proliferation Phase (We will rebuild)
This phase lasts 3 to 24 days, replacing and or repairing lost or damaged tissue.
โ— Granulation โ€“ Growth Factors and Fibroblasts assist with collagen formation
โ— Contraction โ€“ Fibroblasts containing contractile proteins pull the wound edges together, and the size of the wound becomes smaller
โ— Epithelialization โ€“ The epidermis is restored in this stage

3. Remodeling Phase (Let's reinforce)
โ— Final phase in wound healing and lasts from 21 days to 2 years
โ— Collagen synthesis is ongoing to strengthen the tissue
โ— Healed tissue will always be at risk for a breakdown as skin tensile strength is no longer 100%
