External aortic compression: buying time to save lives in obstetric hemorrhage: External Aortic Compression for the Control of Obstetric Hemorrhage. A. Frontal View and B. Lateral View of External Manual Aortic Compression: The operator should position themselves next to the patient and use a step stool to acquire a height that allows them to place their dominant arm extended over the patient's navel at a 90-degree angle to the stretcher. The operator's body position is the same as used for chest compressions during cardiopulmonary resuscitation, with the elbow extended, and the back and knees straight. The operator should not attempt to exert muscular force on the patient, but simply rest the weight of their torso on the patient's abdomen. https://ow.ly/NEfe50TKjs1

THEY ARE PEOPLE, THEY GET TIRED

Working night shifts can be challenging, especially in healthcare where patient safety and care are paramount. Here are some strategies to help medical personnel stay alert and avoid fatigue-related errors during night shifts:

Pre-Shift Preparation:

1. Get Adequate Rest:

Ensure you have sufficient sleep before your night shift. Aim for 7-9 hours of quality sleep during the day.

2. Healthy Eating

Consume a balanced meal before your shift to maintain energy levels. Avoid heavy or greasy foods that can cause lethargy.

3. Hydration

Stay hydrated throughout your shift with water or other hydrating fluids. Avoid excessive caffeine and sugary drinks.

During the Shift:

1. Stay Active:

Engage in light physical activity, such as stretching or short walks, to keep your blood flowing and stay alert.

2. Take Breaks

Use your breaks to rest, eat healthy snacks, or engage in activities that help you relax and recharge.

3. Caffeine Moderation:

Use caffeine strategically to boost alertness, but avoid excessive intake that can lead to jitters or a crash later.

4. Bright Lighting

Exposure to bright light can help increase alertness. Ensure your workspace is well-lit.

Strategies to Stay Alert:

1. Stay Engaged

Focus on your tasks and patient care. Engaging with your work can help keep you alert.

2. Team Communication

Communicate regularly with your team to ensure everyone is aware of patient statuses and any concerns.

3. Prioritize Tasks:

Organize your tasks efficiently to manage your workload and reduce stress.

Portal hypertension is an increase in pressure within a system of veins called the portal venous system. Veins from the stomach, intestine, spleen, and pancreas join the portal vein, which branches off into smaller vessels that travels through the liver. If the vessels in the liver become blocked, blood cannot flow properly through the liver. As a result, high pressure in the portal system develops. This increase in pressure can lead to large, swollen veins in the esophagus, stomach, re**um, or umbilical area

Alzheimer's Disease

AD is a neurodegenerative disorder of uncertain cause and pathogenesis that primarily affects older adults and is the most common cause of dementia.

DEMENTIA, based on the criteria:

Significant acquired cognitive impairment that interferes with independence and affects at least one of the following:

- Learning and memory
- Language
- Executive function
- Complex attention
- Perceptual-motor function
- Social cognition

Dementia in AD has an insidious onset and is progressive. Treatments are available that ameliorate some symptoms, but there is no cure or disease-modifying therapy (treatment that slows the course of the illness) currently available, and the disease inevitably progresses. There are no survivors.

The prevalence of AD doubles every 5 years over the age of 65.

Early-onset AD (onset of symptoms < 65 years of age) is unusual, and many of these patients present for evaluation due to concerns about job performance.

CAUSE: the exact mechanisms remain unclear and are the subject of on-going research, but all forms of AD seem to involve excessive amounts of amyloid beta peptides in the brain--from overproduction and/or decreased clearance. Abnormal clusters of these chemically “sticky” proteins are called "plaques," which can clump together as tangles that are prime suspects in cell death and tissue loss.

Hydrocephalus is the buildup of fluid within the ventricles. The excess fluid increases the size of the ventricles causing pressure in the brain. There are 2 types of hydrocephalus congenital and acquired.
Congenital hydrocephalus is present at birth. The causes include genetic problems and problems with how the fetus develops. An unusually large head is the main sign of congenital hydrocephalus.
Acquired hydrocephalus and can occur at any age. The causes can include head injuries, strokes, infections, tumors, and bleeding in the brain.
Symptoms include:
-Headache
-Vomiting and nausea
-Blurry vision
-Balance problems
-Bladder control problems
-Thinking and memory problems

An amazing image of the "Heart Strings."
The chordae tendinae are strong, fibrous strings that connect the papillary muscles to the tricuspid and mitral valve in the heart. During atrial systole, blood flows from the atria to the ventricles down the pressure gradient. Chordae tendineae are relaxed because the atrioventricular valves are forced open. When the ventricles of the heart contract in ventricular systole, the increased blood pressures in both chambers push the AV valves to close, preventing backflow of blood into the atria. Since the blood pressure in atria is much lower than that in the ventricles, the flaps attempt to evert to the low pressure REGIONS. The chordae tendineae prevent prolapse by becoming tense thus pulling the flaps, holding them in closed position
