Learn from the past to protect the future.

Today’s SHE focus is on reviewing a past incident report—not to assign blame, but to understand what went wrong and how we can do better. Every incident, near miss, or unsafe condition is a learning opportunity.

🔍 What was learned?
✔️ The root cause of the incident—not just the outcome
✔️ Gaps in procedures, training, or communication
✔️ Early warning signs that may have been overlooked

🛠️ What improvements were made?
✔️ Updated safe work procedures and controls
✔️ Additional training and awareness for workers
✔️ Improved inspections, supervision, or equipment safeguards

🔧 Tip of the Day:

Ensure engineering controls like machine guards or ventilation systems are functioning as intended.

✔️ What to Do:

🔍 Inspect Regularly: Check that machine guards, barriers, enclosures, and ventilation systems are properly installed and not damaged.

🛠️ Test Functionality: Make sure systems are working effectively—ventilation should remove fumes, guards should prevent contact with moving parts.

🚫 Never Bypass Controls: Do not remove or disable engineering controls for convenience.

📢 Report Faults Immediately: If something isn’t working as designed, notify your supervisor or safety officer.

🛑 Remember:

Engineering controls are designed to remove or reduce hazards at the source—making them more effective than relying on PPE alone.

Always ensure your Personal Protective Equipment (PPE) is available, appropriate, and in good condition before starting any task.

✅ What to Do:

🔍 Inspect Before Use – Check for cracks, tears, missing parts, or wear and tear.
🎯 Use the Right PPE – Make sure it matches the specific hazard (e.g. gloves, helmets, goggles, respirators).
🔄 Replace When Damaged – Faulty PPE offers false protection.
📚 Know How to Use It – Incorrect use reduces effectiveness.

Before starting any hazardous task, take time to review your risk control measures. Ask yourself:

* Are the controls suitable for the task?
* Are they practical and easy to follow?
* Has everyone been trained and informed?
* Are they actively being used on site?

Effective risk controls only work when they are understood, implemented, and monitored. Never assume safety—verify it.

Zero Harm begins with awareness, responsibility, and action.

The AIOSHE Institute’s Executive Director’s Council (IEDC) is pleased to announce the appointment of Mr. Tranos Matimba as Principal Officer of the Institute.

Mr. Matimba is widely recognized for his expertise within Zimbabwe’s education and training sector. He holds a Master’s Degree in Education Management, a Bachelor’s Degree in Educational Administration, Planning and Policy Studies, and a Certificate in Education.

As the Principal Officer, Mr. Matimba will provide strategic leadership and direction for the Institute’s operations, with a particular focus on enhancing education and training services. He is committed to upholding the highest standards in educational delivery, leveraging his extensive experience to benefit both learners and the broader industry in support of the zero harm initiative.

His professional and technical credentials include a Diploma in Personnel Management from the Institute of Personnel Management of Zimbabwe (IPMZ), a Diploma in Transport and Logistics from the Zimbabwe Institute of Management (ZIM), a Diploma in Project Monitoring and Evaluation from the Training and Development Institute of Technology (TDIT), Train of Trainers (TOT) Certification from Zimbabwe Institute of Management ZIM, and Defensive Driver Trainer certification, among others.

With over 30 years of experience, Mr. Matimba has held distinguished roles in the education and training sector, notably serving at the Traffic Safety Council of Zimbabwe where he progressed from Traffic Safety Officer to Acting Regional Manager. This tenure reflects his strong leadership and comprehensive understanding of traffic safety management.

Additionally, Mr. Matimba served as Provincial Vice Chairman of the Red Cross and represented Zimbabwe in international Red Cross First Aid Competitions in South Africa and France, earning gold medals. These achievements demonstrate his dedication and excellence in first aid and emergency response training.
