If you or someone you love needs a little extra help at home, you may be eligible for government funding to stay in your own home longer. Many older Australians want to stay independent and close to family and friends, but you might be wondering - how can I get a Home Care Package and where do I start? 🏡

At PlanCare, we’re here to provide care that fits your needs, giving you more choice and extra support so you can live safely and comfortably at home! 🩵

Want to know if you're eligible for a Home Care Package? Read more about your options and how you can stay independent at home for longer here 👉🏻 https://plancare.com.au/how-to-access-care-at-home

This , we'll be talking about the Income Tested Fee (ITF) which is a fee you may be required to pay towards your Home Care Package, depending on your income and assets.

When you receive a Home Care Package, you will need to undergo an Income Assessment, to determine if you should contribute to the cost of the supports provided.

Once completed via the My Aged Care website, Services Australia will notify you and your provider of the amount you will need to pay towards your care.

In the new Support at home (SAH) program, these will be called Client Contribution fees. They work similarly to an ITF but are now required for all participants. The amount a person contributes will vary depending on the type of services they receive and the results of their income assessment.

If you'd like to know more, read our blog about ITFs here 👉https://plancare.com.au/income-tested-fees

For people living with dementia, everyday tasks like making a cup of tea, finding the bathroom, or getting dressed can slowly become more difficult.

If someone you care for is showing signs of memory loss or confusion, one of the most helpful first steps is to declutter. This can really help maintain a calm and safe environment at home🩵🏡

There are other simple things you can also do to make life a little easier and safer at home. Dementia Australia has some very helpful videos and examples on their website to guide you! They show what works well and how other families have made their homes more supportive for a loved one with dementia.

When it comes to getting the most from your Home Care Package, having choice and control is key. One way to take greater ownership of your supports is by choosing self-management.

Though it comes with more work and responsibilities, it does offer:
✅ Greater choice
✅ More control over your funds
✅ Lower costs
✅ Feeling of empowerment

Self-management works well for people who value independence and decision-making, are comfortable, have the time and flexibility to coordinate services themselves and doing a bit of admin work.

Under the new Support at Home program starting 1 November, older Australians will still be able to receive a wide range of services, including personal care, domestic assistance, meal preparation, transport, home maintenance, nursing, and allied health. All designed to help people live independently in their own homes for longer🩵

The three new categories of services will be:
✅ Clinical
✅ Independence
✅ Everyday living

Some services come with eligibility rules or service limits, meaning they won’t
automatically be included in every care plan or available to all clients.

In this week's we'll be shining some light to the Commonwealth Home Support Program or CHSP! 🩵

CHSP offers entry-level support to help older Australians stay safe and independent at home. It’s ideal for those who need a bit of help with everyday tasks like household chores, meals, personal care, and transport, but don’t yet require a full Home Care Package (HCP).

CHSP is funded by the Australian Government and provided through approved organisations. While PlanCare isn’t a CHSP provider, we are an approved Home Care Package provider, offering older Australians more personalised and long-term care options at home.

The CHSP will transition into the Support at Home (SAH) model in 2027, aligning with the broader reforms to make in-home care simpler and more effective.

A shift note is more than just paperwork - it describes what occurred during the support session.

Shift notes help track a client’s wellbeing, ensure safety and support accurate invoicing to providers. A clear note means better continuity and quicker follow-up when it matters.
