For this week's , we'll be talking about the Home Care Package or 'HCP' which is an Australian Government program that provides support to older people with more complex needs to help them to stay at home 🏡

As you may have or may not have heard already- the HCP program will soon be replaced by the Support at Home (SAH) program on 1st November 2025 into a much simpler, easy-to-use system. With the right support and more choice in your care, you can stay independent and enjoy a deeper connection to what matters most – all from the comfort of your own home💙

When you receive a Home Care Package, you get to decide how your care is managed. There are two main ways to do this: self-management and full-management and each has its benefits depending on what suits your lifestyle and preferences.

💛Self-management:
Allows for more control and flexibility, and often lower fees, but requires time, confidence, and a willingness to be more hands-on.

🙋🏻‍♀️Full-management:
This is ideal for people who prefer a hands-off approach or have complex care needs and want support navigating the aged care system.

Not all home care providers offer both options, so be sure to ask before choosing a provider. At PlanCare, we offer both services so you have the flexibility to switch between self-management and full-management at any time without any issues if you’re unsure which is right for you.
