We are currently looking for an enthusiastic and engaging Registered Nurse to join our team!

We are looking for a Nurse who can provide quality care to a diverse mix of patients through:
* Chronic Disease Management including Care Plans coordination
* Health Assessments
* Identifying QI activities to engage our patients with improving health outcomes
* Identifying & contributing to the development of our patients educational needs
* Immunisations & Vaccinations
* ECG’s & Spirometry
Essential Criteria:
* Current unrestricted Nurses Registration with AHPRA
* Experience with the Australian HealthCare system and Medicare item numbers
* Experience in contributing to CDM & HA
* Current CPR certification

* Previous experience as Practice Nurse or a similar role
* Excellent interpersonal & communication skills
* Proficiency with IT systems
* Ability to work independently and as a part of a team
* Caring & collaborative approach to patient care
* Proven multi-tasking and time management skills.
