About Us:

We're a small but mighty crew who genuinely care. We’re now looking for a full-time NDIS Support Coordinator who is ready to hit the ground running and provide high-quality, person-centred support to our growing client base.
The Role:
As a Support Coordinator, you'll work closely with NDIS participants to understand their funding allocations, coordinate their supports, and help them navigate the NDIS with confidence. This is a remote role, but you’ll be supported every step of the way by a highly engaged and collaborative team.
Key Responsibilities:
🔹Support participants to implement their NDIS plans in a meaningful way
🔹Build the capacity of participants to navigate services and make informed decisions
🔹Establish and maintain strong relationships with participants, families, providers, and stakeholders
🔹Coordinate and monitor the delivery of supports to ensure outcomes are achieved
🔹Prepare professional documentation, including progress reports and case notes
🔹Maintain regular contact with participants to provide ongoing support and guidance
What You’ll Need:
✅ Strong knowledge of the NDIS, including funding categories
✅ A minimum of 12 months experience within the disability industry
✅ Exceptional communication and organisational skills
✅ Confidence working independently and managing a caseload
✅ Current or a willingness to obtain an NDIS Worker Screening Check, WWCC, and Police Clearance
✅ Relevant qualifications in community services, social work, or similar field (preferred but not required)
