When we first met Sam in August 2024, he was feeling unheard, socially isolated, and unsure who he could trust. He only had 1 therapist in place at the time and weren't really making a real difference for him.

We knew Sam deserved more! 💛

✨ With his goals and strengths at the center, we:
✅ Helped him build a routine that worked for him and gave his days more structure
✅ Connected Sam with an alternative Occupational Therapist and Exercise Physiologist who listened and tailored supports to his needs
✅ Completed a specialised driving assessment and secured plan funding so he could start learning to drive 🚗
✅ Built a trusted team of support workers around him for ongoing encouragement, connection and community access
✅ Helped Sam secure a volunteering role at Scitech—a huge step towards his employment aspirations! 🌱

Now, Sam is exploring employment opportunities, including with the Bureau of Meteorology, because we believe the sky is the limit for our participants 🌤️

🌱Rachael’s Journey: Why People Come First🌱

When Rachael came to us in January 2024, she was at the very beginning of her NDIS journey—hopeful but unsure where to start.

We supported Rachael through every step of her application process, attending her initial and secondary meetings to make sure her voice was heard and her needs understood. When her plan was approved—without funding for Support Coordination—we didn’t let that stop us.

Over the past 18 months, we’ve worked alongside Rachael to help her build the life she imagined:

We're a small but mighty crew who genuinely care. We’re now looking for a full-time NDIS Support Coordinator who is ready to hit the ground running and provide high-quality, person-centred support to our growing client base.

The Role:
As a Support Coordinator, you'll work closely with NDIS participants to understand their funding allocations, coordinate their supports, and help them navigate the NDIS with confidence. This is a remote role, but you’ll be supported every step of the way by a highly engaged and collaborative team.

Key Responsibilities:
🔹Support participants to implement their NDIS plans in a meaningful way
🔹Build the capacity of participants to navigate services and make informed decisions
🔹Establish and maintain strong relationships with participants, families, providers, and stakeholders
🔹Coordinate and monitor the delivery of supports to ensure outcomes are achieved
🔹Prepare professional documentation, including progress reports and case notes
🔹Maintain regular contact with participants to provide ongoing support and guidance

What You’ll Need:
✅ Strong knowledge of the NDIS, including funding categories
✅ A minimum of 12 months experience within the disability industry
✅ Exceptional communication and organisational skills
✅ Confidence working independently and managing a caseload
✅ Current or a willingness to obtain an NDIS Worker Screening Check, WWCC, and Police Clearance
✅ Relevant qualifications in community services, social work, or similar field (preferred but not required)
