We are seeking support workers to add to our team NOR. These positions are outreach role—support is provided in home and out in the community. The positions require you to working independently in the community and as part of a team.

Wanneroo area
To assist our client with psychosocial disabilities to achieve their goals and build independence.
• Day-to-day support in the home
• Developing life skills (e.g., time management)
• Supporting social connection and community participation
• Transporting participants to appointments and community activities

Balcatta
We are seeking Support Workers to assist our client in daily activities.
• At home day-to-day support
• Developing life skills (e.g., social interaction)
• Supporting client in personal care, medication, manual handling, social connection and community participation

* Seeking a young and fun female Support Worker to work with a 12-year-old
female client with ASD.
�� Greenwood / Balga / Girrawheen
* Seeking a mature female Support Worker to assist clients with their daily
activities.
�� Kingsley
* Seeking a support worker to assist client with daily activities and community
access, while also supporting the development of her cognitive skills. An
understanding of ASD and ADHD is essential.

Essential Requirements:
✔ Certificate III in Individual Support (or relevant qualification)
✔ Current Police Clearance
✔ Valid Driver’s Licence & Reliable Vehicle
✔ Working With Children Check (WWCC)
✔ Current First Aid & CPR

Family is looking for a Team Leader/Support worker to add to the team. 13 year old boy with ADHD/Level III Autism. Easy-going boy needs direction and prompting.

Client Preference:
someone who is young preferably male but shows maturity and is firm but fair.
someone who has similar interests: coding, gaming, plays an instrument, baking, cooking, bounce, swimming, tennis

Location: Between Butler and Two Rocks

Essential:
- Drivers License and Vehicle with Comprehensive Car insurance + roadside assistance
- WWCC
- Police Clearance
- NDIS Worker's Screening check
- First Aid/CPR (some knowledge of diabetes but not essential)

Chakra Care is seeking an enthusiastic Team Leader to support our clients starting February 2025. The suitable candidate will have substantial knowledge and experience in working with challenging behaviours and mental health disabilities. This team is active between Joondalup and Two Rocks.
We are also seeking support workers in the Claremont and Balcatta area with personal care and complex care experience.
You are requested to provide the relevant checks such as police clearance, WWC, NDIS clearance, Cert III in Individual Support, Medication Comp etc
