👣 Accessory Navicular Pain – Will Shockwave Help?

If you (or your child) have pain on the inside of the foot, especially with flat feet, an accessory navicular could be the cause. This small extra bone can place extra strain on the tibialis posterior tendon, leading to ongoing discomfort.

💥 What about shockwave therapy?
Shockwave can be helpful in some cases—but it’s not always the first step.

✅ It may help if:
• Pain has been ongoing for months
• The tendon is irritated (tendinopathy)
• Other treatments haven’t fully resolved symptoms

⚠️ It’s less effective if:
• The area is very inflamed or acute
• The bone itself is the main source of irritation
• Foot mechanics aren’t being supported

👟 Best approach?
We usually start with:
• Supportive footwear
• Custom orthotics (to reduce strain)
• Strengthening exercises (especially tibialis posterior)

A flat foot (pes planus) doesn’t just affect how you walk—it can impact your entire body.

When the arch collapses:
➡️ The foot rolls inward (pronation)
➡️ The leg rotates inward
➡️ The hips and pelvis shift

Toe walking might seem cute at first, but if it continues around age 2, it can actually slow a child’s development.

When children walk on their toes, they miss out on important movement patterns like:

Heel-to-toe walking (which helps balance)
Proper muscle use in the legs and feet
Stable standing, squatting, and climbing

Should You Wear Orthotics at the Gym? 👟

This is one of the most common questions I get… and the honest answer is: it’s largely personal preference.

There’s no one-size-fits-all rule, but here are a few things to consider:

✔️ Wear them if:
• You have foot pain, plantar fasciitis, or a known injury
• You rely on them for daily comfort and support
• You’re doing high-impact activities (running, HIIT, jumping)

✔️ You might skip them if:
• You’re doing strength training (e.g. squats, deadlifts) and want better ground contact
• You feel more stable and connected without them
• Your body tolerates it well pain-free

💡 Key takeaway:
Your body will usually tell you what it prefers. Comfort, stability, and injury prevention matter more than strict rules.

If you’re unsure, try both options and see how your body responds — or get tailored advice from your podiatrist.

👣 Why do we get hard corns on our feet?

Ever feel like you’re walking on a tiny stone? That could be a hard corn.

Hard corns are caused by repeated pressure and friction—usually from shoes or the way your foot moves. Your skin tries to protect itself by thickening, but instead forms a hard central core that presses into deeper tissue… and that’s what makes them painful.

🔎 Common causes:
• Tight or rubbing footwear
• Toe deformities (like hammer toes)
• High pressure areas under the foot
• Changes in walking patterns

📍 Where do they show up?
• Tops of toes
• Between toes
• Ball of the foot
• Outside of the little toe

⚠️ Important:
Corns aren’t the real problem—they’re a sign of too much pressure in one spot.

Simply cutting them off = they come back ❌
Reducing the pressure = long-term relief ✅

💡 What actually helps?
• Properly fitted shoes
• Pressure offloading (padding or orthotics)
• Regular podiatry care

If corns keep coming back or are painful, it’s worth getting them properly assessed—especially if you have diabetes or sensitive feet.

30 years of practice on 26/2/26. So what have I learnt during that time.

1. Most people come to me after a few months rather than straight away. Ultimately costing more money treat the problem.

2. My love and passion for the profession is still here.

3. I keep learning… current interests are Autism, toe walking.

4. I use my Law degree to help with AT Assessments and report writing.

A well worn orthotic.. if your orthotics are uncomfortable or don’t feel supportive enough, your body will let you know with symptoms of pain and discomfort.

It might be a return of the original pain such as heel pain or a new pain. The quicker you make the appointment, the quicker the resolution of pain.
