In recognition of the passing of the WA legislation improving access to fertility treatments for the LGBTQIA community today’s Thursday Thought looks at what reciprocal IVF is and important measures that should be taken to ensure a successful pregnancy outcome. Please note this post uses gendered language but means no disrespect to those people who do not identify as a binary gender and who have the same rights to parenthood and fertility treatments as all members of our community. 🌈

A wonderful night at the WA Fertility Nurses Australasia Dinner meeting, collaborating with a wonderful group of passionate and dedicated Fertility Nurses from fertility units all across Perth and WA. A great discussion was had about the impact IVF treatment can have on placental development and implications later in pregnancy as well as the amazing techniques and innovations in ultrasound that are allowing us to detect, diagnose and treat multiple issues that may affect our patients on their fertility journey.

Finishing up our discussion on uterine NK cells I look at how we can treat abnormal levels, but more importantly the limited evidence for most of the treatments available- and the importance of individualized treatment in this largely evidence-free zone.

Uterine Natural Killer Cells levels can be measured to determine if an immune imbalance may be playing a role in recurrent implantation failure. Today we discuss how we test for uNK cell levels to determine if your levels may be abnormal. Through tracking your cycle and sampling your endometrium, at the time an embryo would be hoping to implant, allows us to compare your levels of NK cells to population data telling us if it is low, normal or high.

On today’s Thursday Thought I discuss the controversial topic of uterine Natural Killer cells, looking at what role they play in fertility and when you might need to be investigated for abnormal levels. Natural Killer cells are vital for healthy implantation but there is limited evidence as to what role abnormal levels of NK cells may play in recurrent implantation failure or recurrent miscarriage and so there is debate about if we should be testing levels and what to do with an abnormal result.

Long post ahead… not a Thursday Thought- more like a Sunday Seminar- be warned!
Two years on from the loss of our beloved husband and Dad today has been spent reminiscing over the many treasured memories we have of him and doing something he would have loved- we climbed Bluff Knoll, just the 3 of us, feeling his love and presence with us every step. When Ben was first diagnosed with the aggressive cancer that would take him just 9 months later, my GP told me everyone faces a great challenge in their lives and this was probably going to be ours. I think a lot about this comment and I think there is so much that can be taken from it. We can do very little to control the cards or challenges we get dealt in this one short life- what we can do is control how we respond and what we do when faced with these challenges. When cancer came and turned my family’s life upside down I decided we could let it define us, we could stay sad and grief stricken or we could take control- realize this is what life had thrown at us and work out how we could make the best of it. Nothing will bring Ben back but we can instead live our lives in a way that would make him proud, every single day. Two years on I feel I am so much calmer than before this happened, I have a better life-work balance, am a much more present parent and day to day find so much joy in the littlest things. I no longer stress or sweat over the small stuff- the big picture has definitely become clearer! Peace and calm comes with accepting things that you cannot control but realizing you can control how you react and move through them. When your big life challenge comes know you can and will get through it. Don’t let it control and define you, only you can take on life’s challenges and find the positive and the way forward- that is the real challenge and the key to getting through. Happy, peaceful Sunday x

Egg collection Friday was spent with my colleague and friend, anaesthetist Dr Yelena Hoppe- a brilliant anaesthetist who has the most beautiful bedside manner with patients, particularly during such an anxiety producing procedure like an egg collection. I’ve known Yelena since med school and I even had the absolute pleasure of delivering her babies. About Dr Langdon

Dr Fiona Langdon is a Specialist Obstetrician and Gynaecologist caring for women at St John of God Hospital, Subiaco.

Dr Langdon graduated medical school with Honours from the University of Western Australia in 2005 before completing her internship and residency at Royal Perth Hospital and the University College Hospital in Galway, Ireland. During this time she worked in a variety of medical specialities allowing her to gain a better knowledge of medical disorders and their effect on pregnancy.

She undertook 6 years of specialist training in Obstetrics and Gynaecology at King Edward Memorial Hospital, including Fellowships in Maternal Fetal Medicine and Fertility and Reproductive Medicine. She has worked extensively with the Preterm Birth Prevention Team at King Edward Memorial Hospital and has a special interest in preterm birth prevention and caring for women with a history or risk of preterm birth.

Fiona has a particular interest in high risk obstetrics and particularly enjoys the challenge of assisting women with complex medical disorders through their pregnancy.
