Are you a senior or caregiver and finding it a struggle to cope with the activities of daily living?

Consider reaching out to Continuing Care Nova Scotia (Nova Scotia Health) at 1-800-225-7225. The NS Continuing Care Coordinators are there to help you navigate the resources and supports available through home care and long term care.

You can make a referral on behalf of yourself or as a caregiver for someone else. Leave your name, contact number, and a brief description of the reason for your call. Expect your call to be returned in 2-3 days. It is helpful to have the name, contact details and health card number for the person being referred. If that person is not you, then you will need that person’s consent.

If you are unsure about reaching out to Continuing Care, book a session with counsellor Nancy Malek at the clinic to discuss your needs and to determine the information that will be most helpful to have on hand when you do talk to Continuing Care NS.
