Phase Two of West Bedford Transitional Health introduces four new Transition‑to‑Home Apartments designed to help patients build confidence before discharge. These spaces replicate a home environment, complete with a kitchen and laundry area, giving patients a safe place to practice daily tasks and strengthen their independence.

It’s another way we’re defining transitional care in Nova Scotia, because building confidence is just as important as building physical strength.

When Susan Forrest came to West Bedford Transitional Health (WBTH), she was recovering from her second leg amputation. Her restorative action plan included strengthening her upper body to support her in daily activities. An intimidating challenge, but one Susan credits to having courage and support from her health team at WBTH.

“Courage is the main thing. You have to have courage, and you have to work with your team, whether it be Occupational Therapists or Physiotherapists, Nurses, Transitional Care Assistants – work with them.”
