Years ago, I treated an inmate injured in jail.

Then he added:
“Give me a name. Your problem will disappear.
Even from the inside.”

I refused.
But I never forgot the moment.

Because it revealed something unsettling:
sometimes loyalty and accountability are clearer
in dark places
than in institutions built on oaths.

In medicine, we swear to serve truth.
In the real world, complaints, image, procedures,
and reputation often replace it.
