This patient previously underwent a closed rhinoplasty elsewhere and was left with an irregular dorsum, a drooping nasal tip, and significant deviation. Fortunately, her septum had not been harvested, allowing us to use her own septal cartilage for structural support in this revision.

Using Ultrasonic Piezo technology, I was able to precisely smooth and refine the dorsum, correct the tip deviation, and address the drooping appearance by improving the hanging columella and retracted alar rim.

At just 1 month post-op, she is still very early in healing journey but I am so pleased with her result so far ✨

Cette patiente avait déjà subi une rhinoplastie fermée ailleurs et présentait une bosse dorsale irrégulière, une pointe tombante ainsi qu’une déviation marquée. Before the first incision is made, meticulous measurements and markings are performed to determine exactly how much skin can be removed safely. Precision is everything. Excessive removal can compromise eyelid closure and lead to complications such as lagophthalmos, which is why I re-measure and triple check every marking before proceeding.

Once the procedure is complete, the incision is carefully sutured within the natural eyelid crease. Over the following months, the scar softens and fades, becoming nearly imperceptible in most skin types. The result is a brighter, lighter, more rested appearance that still looks entirely natural ✨

Avant la toute première incision, des mesures et des marquages minutieux sont effectués afin de déterminer précisément la quantité de peau pouvant être retirée en toute sécurité. Un magnifique dévoilement avec ma merveilleuse patiente, à seulement 48 heures de son lifting profond du visage et du cou 🥹✨

Après une perte de poids importante, elle présentait un relâchement au niveau du cou, une situation fréquente lorsque la peau et les structures profondes perdent leur soutien. A touching reveal from my wonderful patient, just 48 hours after her Deep Plane Face and Neck Lift 🥹✨

After significant weight loss, she was left with laxity in the neck, something we see quite often when the skin and underlying structures lose support. With a deep neck lift, I was able to reattach the platysma muscles and carefully reduce the submandibular glands, two key steps in restoring smoothness and definition to the neck contour.

Sometimes rib cartilage is necessary even in a first-time rhinoplasty.

There are two main reasons:

1️⃣ The septum isn’t usable or isn’t there.
In some patients, the septal cartilage has been significantly damaged or destroyed. This can happen from prior trauma, intranasal drug use, or even chronic nose picking. If there isn’t enough healthy septal cartilage to safely build support, we need another reliable source.

2️⃣ The septal cartilage isn’t strong enough.
Not all cartilage is created equal. Certain patients naturally have softer, thinner septal cartilage. In many Asian and African American patients, we also see thicker skin paired with weaker underlying cartilage.

Thicker skin requires a stronger structural framework underneath to properly support refinement and long-term stability. In these cases, rib cartilage provides the strength and durability needed to build a nose that will hold its shape over time.

The 2 most common rhinoplasty cases I choose to decline are:

1️⃣ Highly complex revision cases
This often involves patients who have undergone multiple rhinoplasties. With repeated surgery, risks increase, including thinning skin, weakened support, scar tissue, structural irregularities, and compromised blood supply. When the risk outweighs the potential benefit, proceeding is not in the patient’s best interest.

2️⃣ Unrealistic expectations
This is, by far, the most common reason. My goal in every case is for the patient to feel genuinely happy and confident with their result. If I believe that a patient’s expectations cannot realistically be met, I feel it’s my responsibility to be honest.

Sometimes, the best decision is knowing when to say no. I would always rather decline a case than promise an outcome that isn’t achievable.

Les deux types de cas de rhinoplastie que je choisis le plus souvent de refuser sont :

1️⃣ Les cas de révision très complexes
Il s’agit souvent de patients ayant déjà subi plusieurs rhinoplasties. Avec les chirurgies répétées, les risques augmentent : amincissement de la peau, affaiblissement du soutien structurel, tissu cicatriciel excessif, irrégularités de la structure nasale et altération de l’apport sanguin. Lorsque les risques surpassent les bénéfices potentiels, aller de l’avant n’est tout simplement pas dans l’intérêt du patient.

2️⃣ Des attentes irréalistes
C’est, de loin, la raison la plus fréquente. Mon objectif, dans chaque cas, est que le patient se sente réellement satisfait et en confiance avec son résultat. Si je crois que les attentes ne peuvent pas être atteintes de façon réaliste, il est de ma responsabilité d’être honnête.

Parfois, la meilleure décision est de savoir dire non. Je préférerai toujours refuser un cas plutôt que de promettre un résultat qui n’est pas réalisable.

Reconstruction nasale complète avec GREFFES DE CÔTE 🦴👃

À seulement 1 mois post-op, cette patiente présentait auparavant un affaissement nasal complet, entraînant une déformation en nez en selle. Dans son cas, cet affaissement était secondaire à une perforation quasi totale du septum nasal, liée à une polytoxicomanie intra-nasale, laissant le nez sans soutien structurel adéquat.

Lorsqu’une reconstruction nasale complète est nécessaire, le cartilage costal demeure la référence absolue. Il offre la solidité, la stabilité et la durabilité requises pour recréer une charpente nasale à la fois fonctionnelle et naturelle.

Je suis extrêmement satisfaite de son évolution précoce. Même à ce stade de la guérison, son nez s’intègre déjà harmonieusement à ses traits, comme si elle était née avec.

Total Nasal Reconstruction using RIB GRAFTS 🦴👃

At just 1 month post-op, this patient previously presented with a complete nasal collapse resulting in a saddle nose deformity. In her case, the collapse was secondary to a near-total perforation of the nasal septum related to intranasal polysubstance use, leaving the nose without adequate structural support.

When a complete nasal reconstruction is required, rib cartilage remains the gold standard. It provides the strength, stability, and durability needed to recreate a nasal framework that is both functional and natural in appearance.

I am extremely pleased with her early progress. Even at this stage of healing, her nose already integrates beautifully with her facial features, as if she could have been born with it.

2 months post Deep Neck Lift for my 37 year old patient 😍

A poorly defined jawline is often assumed to be caused by excess fat, but that isn’t always the case. While liposuction can address superficial fat under the chin, it cannot correct deeper structural issues.

In patients with separated or weakened platysma muscles, true definition comes from addressing the deeper layers of the neck. One of the key advantages of a deep neck lift is the ability to reposition the musculature and restore support, creating a smoother, more sculpted, and longer-lasting result.

→ Repaired and reattached the platysma muscles
→ Trimmed excess digastric muscle and reduced the submandibular glands through a small incision under the chin
→ Elevated the skin and muscle through small incisions hidden around the earlobe

Although she is still experiencing some residual swelling in the submental area, her results already show a noticeably sharper jawline, reduced jowl laxity with a beautiful V-shape from the front, and a well-defined neck–chin angle.

This patient underwent an Ultrasonic Rhinoplasty combined with a chin implant to restore balance across her facial profile.

In her case, the focus was not on changing individual features, but on how they relate to one another. By subtly refining the nose and enhancing chin projection, we were able to soften the overall contour and create a smoother, more cohesive profile from forehead to chin.

An extended chin implant was chosen to address both projection and the pre-jowl sulcus, helping to elongate the jawline and improve the lower face transition.

Cette patiente a subi une rhinoplastie ultrasonique combinée à un implant du menton afin de rétablir l’équilibre de son profil facial.

Dans son cas, l’objectif n’était pas de modifier chaque trait individuellement, mais plutôt de travailler l’harmonie entre eux. En affinant subtilement le nez et en améliorant la projection du menton, nous avons adouci le contour global et créé un profil plus fluide et cohérent, du front jusqu’au menton.

Un implant du menton étendu a été choisi afin de corriger à la fois la projection et le creux pré-bajoue, ce qui a permis d’allonger la mâchoire et d’améliorer la transition du bas du visage.

Bien qu’elle soit encore au début du processus de guérison et présente un léger gonflement résiduel, à seulement 1 mois post-opératoire, son profil apparaît déjà nettement plus harmonieux. Avec le temps, la cicatrisation progressive révélera encore davantage de finesse ✨

How do I determine if a patient would benefit from a chin implant? 💭

There are two key factors I assess during the consultation:

1️⃣ Projection
For women, the chin ideally sits about 1–2 mm behind the lower lip. For men, the chin can align with the lower lip to maintain a stronger profile.

2️⃣ Pre-jowl sulcus
This helps determine the type of implant best suited for the patient. When the pre-jowl sulcus is hollow, an extended chin implant is often preferred. This type of implant includes wings on either side that help fill in the area and smooth the jawline transition.

For patients who want to explore their options before committing to surgery, filler can also be used as a temporary, non-surgical way to assess balance and projection.

Ultimately, the goal isn’t just a stronger chin. It’s creating harmony with the rest of the facial features and achieving a result that looks natural and proportionate.

Comment déterminer si un patient bénéficierait d’un implant du menton? 💭

Lors de la consultation, j’évalue deux facteurs clés :

1️⃣ La projection
Chez les femmes, le menton se situe idéalement environ 1 à 2 mm en arrière de la lèvre inférieure. Chez les hommes, il peut s’aligner avec la lèvre inférieure afin de conserver un profil plus affirmé.

2️⃣ Le sillon pré-jugal
Ce critère permet de déterminer le type d’implant le plus approprié. Lorsque le sillon pré-jugal est creusé, un implant du menton allongé est souvent privilégié. Ce type d’implant comporte des extensions latérales qui aident à combler la zone et à adoucir la transition vers la mâchoire.

Pour les patients qui souhaitent explorer leurs options avant de s’engager dans une chirurgie, les agents de comblement peuvent aussi être utilisés comme solution temporaire et non chirurgicale afin d’évaluer l’équilibre et la projection.

Au final, l’objectif n’est pas simplement d’avoir un menton plus projeté, mais bien de créer une harmonie avec l’ensemble des traits du visage et d’obtenir un résultat naturel et proportionné.
