I’m thrilled to start seeing patients in the new clinic this week! The new office is at 1081 Carling Avenue, Suite 501, at the corner of Carling and Parkdale, across the street from the Civic Hospital. As always, appointments are booked online, or by referral - check the website for details. Fortunately, there hasn’t been too much of an issue with equipment or supply back orders, so keep your eye posted for additional appointment availability this month!

I’m also excited to announce that I will be sharing the new space with ! Lighthouse Osteopathy is the manual osteopathic practice of Neil Melanson, DOMP, D.Sc.O. Neil has been a physical therapist for over 20 years, and also happens to be my partner in life! We’re super happy to be able to see our respective patients in our shared clinic space. Putting in some elbow grease at the new clinic on this Sunday morning. I have had several patients who were surprised to learn that physicians are responsible for financing and managing every aspect of their clinics in the community, from paying for rent, staff salaries and benefits, insurance, electronic medical record software, and every single swab, speculum, and piece of paper used in the clinic (and more). Taking the step to open my own clinic is definitely exciting, but has required major personal investment of time, energy, and money - no public or private investors.

As a physician who works in the province of Ontario, billing almost all revenue directly through the Ministry of health (OHIP), I am paid for the patient’s visit (usually around $37 for a standard visit which typically lasts 10-20 minutes, with additional fees paid for procedures). The costs of running and managing the clinic, my own salary and that of any staff I have come out of that service fee. When a patient does not show up for a scheduled visit, OHIP will not pay for that missed appointment, and another patient (who might be waiting months to see me) could have been seen.

Anyone who says that physicians shouldn’t think about business, billing or money doesn’t realize that we are small business owners (who receive no training in business) in addition to being skilled clinicians who care for our patients deeply. I love what I do, and am entrepreneurial at heart! It’s important for the community to know how our health care system works, since we all fund it, directly and indirectly.

People don’t talk about menopause. And unfortunately there is a lot of junk online and in print about women’s health to fill the void with myths and quackery. Battling this is part of my job: Every. Single. Day. It’s why patient education is such a huge part of my consultations.

OBGYN Dr. Jen Gunter’s The Menopause Manifesto is NOT one of those questionable sources. Get your hands on this patient-friendly, evidence-based, must-read for women entering into (or well within) the menopause transition. Get a copy for yourself, your friend, your daughter, your mother, or your wife (she will love you for it)!

The truth about condoms… Condoms are NOT the best form of birth control (18% pregnancy rate with typical use), but they are absolutely ESSENTIAL for protection against sexually transmitted infections like chlamydia, gonorrhea, trichomoniasis, HIV, syphilis, viral Hepatitis…

Does getting medical aesthetics treatments make you an ANTI-FEMINIST?

Perhaps you’ve noticed a few fine lines and wrinkles, facial volume loss, uneven skin tone, and have been thinking about having medical aesthetics treatments to maintain or subtly restore your youthful appearance… But then you think: “If I get injections to slow the signs of aging, won’t that make me a BAD FEMINIST?”

Think again:

1. CHOICE IS FEMINIST: Feminism means having choice. This includes the choice of what you do with your body. The Truth About IUD Pain 🥵

Limiting Belief: An IUD insertion will be excruciatingly painful, and not worth the superior contraception and menstrual regulation that many of these devices can provide.

Truth: Social media accounts of IUD insertion “nightmares” likely reflect a minority of patients, however there is no universal standard to address pain with IUD insertions. In my experience as a physician who inserts IUDs on a daily basis, anecdotally, my patients report that the procedure was less painful than expected when the measures below are used to help anticipate and manage pain associated with an IUD insertion.

1. COMPREHENSIVE CONSULTATION: Empowers patients to be fully informed of what to expect before, during, and after the procedure, and for the (potentially) years you may have the IUD. It's a chance for you to ask questions, and for your clinician to learn about your unique health needs and preferences, to support you in making the best contraceptive choice.
2. PAIN MEDICATION:
A) Over-the-counter pain medication about one hour before the procedure if the patient can safely take such medications.
B) Modified cervical block to numb the cervix, using lidocaine (topical or injection)
3. COMMUNICATION & ONGOING INFORMED CONSENT (throughout the procedure), so patients know what is happening next (unless they specifically request not to).
4. ADDRESS ANXIETY & SUPPORT: bring a friend, family member or partner into the room for support. Anti-anxiety medications may be prescribed if appropriate.
5. INSERTER EXPERIENCE: experienced clinicians who inserts IUDs on a regular basis are more likely to be prepared to manage any challenges which may arise.
