Administrative Adherence
o To adhere to the plan of care as determined by the Health & Wellness or Medical Advisor
o To observe and report on resident’s/clients care/service utilizing appropriate Company forms or tools;
o To facilitate residents’, move in and transfers where applicable;
o To respond to resident/client/family concerns and ensure appropriate action taken within decision making a
uthority and or concern communicated to Health & Wellness or Medical Advisor;
o To observe & report to the Health & Wellness or Medical Advisor any concerns regarding client’s safety, including physical environment, implement interventions as directed by his/her supervisor & advocate on client’s behalf to ensure client’s safety is maintained;
o To engage with other agencies involved in resident/client care as directed by the Health & Wellness or Medical Advisor;
o To maintain resident/client information confidentiality;
Personal Support
o To escalate medical emergencies to Health & Wellness or Medical Advisor or emergency medical services as appropriate;
o To assist the resident/client with personal care and hygiene and activities of daily living;
o To observe and report on changes in resident’s/client’s condition to Health & Wellness or Medical Advisor;
o To help / plan and prepare nutritious meals as required;
o To assist with ambulation, mobilization, transferring and adaptive aids;
o To demonstrate understanding of resident/client needs and maximize abilities;
o To provide cognitive, social, emotional, and psychological support to the resident/client/family;
o To assist in maintaining a safe and cleanly environment, including resident’s/clients equipment, belongings, and nursing equipment;
o To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Company Code of Conduct;
